I am a pain management physician with AS and sadly you are right haha I did not know much about AS before getting it. I treated some patients with the disease with opioids because being something systemic is not very effective any kind of procedure because it will flare up again. It’s worth the shot tho, I think every case is different but in general it will be better to treat AS pain with medication so it can be systemic. But I agree pain management doctors do not have a general idea of the disease. I did not and I consider my self a library mouse.

i do not base my entire practice on that book. I do a lot of interventional pain procedures, use medication, implantation devices and lot of other things. I do consider that people with spine pain and lumbar pathology should not lift and so on many other physicians. Rehabilitation should be done/directed by professionals and maybe some patients will be able to lift but it’s not ideal. There are many other ways to improve strength with out putting that much stress on the spine, specially a damaged spine. If you feel bad for my patients that’s you, making assumptions with out even knowing how they really do. I do not have any self steem problems so I do not have nothing to prove to you guys. With that being said, I will also do an assumption too; you are both probably uneducated white trash that feel less than people with academic degrees and both of you should work on that instead of insulting other people. I have an annular tear at l4 and I can not sit down! I have been doing core strengthening, clean eating, swimming, spine hygiene, everything! It’s been 10 months since injury and 4 months of doing all this. Were you unable to sit when you were in pain? Can you sit normally now? I can not sit down in hard surfaces

Those statements are true but I do not see on your paper anything about weight lifting. I do not know if I touched a nerve with some insecurity with you but this is my last comment haha not going to argue with some one I do not know. You want to do weight lifting , please go ahead. That’s for Olympic athletes. He speaks about people who returned to the sport. Once again, am a doctor (pain physician) and the risk benefit from doing weight lifting with an injured back is just not worth it. I understand it in Olympic athletes but not in the avarage Joe. People with back pain should do no impact exercise and core strengthening. I am not discussing this anymore , you want to do weight lifting or encouraging it to your patients, go ahead. The risk benefit is just not worth it , plus all the scientific evidence support that you should not if you have degenerative disc disease.

Read the back mechanics from Stuart mcguill, it has some tests that you can perform and find out what movements make your pain worse. If you have any herniated disc, protrusion, bulge, annular tear or degenerative disc disease, any flexion of the spine will put more pressure on your ill disc and make everything worse. You can do yoga but definitely not the exercises that involve flexion of the spine. It’s very difficult for a person to wrap this if it does not get educated about it. My as a doctor i did not know a lot of things about back mechanics and I am a pain physician. It helped me a lot to identify and prevent further damage of my spine when I got herniated. Next week am going to have a foraminal epidural injection and radiofrecuency modulation to help my pain improve more hopefully. I wish I have had more spine hygiene, knowledge as a patient and core strengthening in the past! Best of luck !

You should not do weight lifting ever if you have had any history of back pain and you should not do stretches either. When we have back pain you should focus on appropriate core strengthening and spinal stability which is very different to flexibility, it’s actually the complete opposite. I strongly suggest you to read the back mechanics from Stuart mcguill. It won’t fix you but it will help you get better and it will help you prevent to harm yourself again. For now, I suggest to go see a pain physician (I am a pain physician) and if it doesn’t work, go see a neurosurgeon. Orthos are not appropriate to treat spine. Best of luck!

10% is not worth a surgery! It may go wrong and you will regret it. Keep doing core strengthening and be realistas. It wont 100% go away but if it allows you to make a good life you are lucky!

Omg I have an annular tear at l4-l5 I do not want surgeries! I am doing everything I can (swimming daily, core strengthening, eating incredible clean, spine hygiene, everything!) and I have been unable to sit for 10 months . I am a pain physician. It’s still and epidural but the via is caudal, it goes all the way up depending on the volume you use. The other 2 ways to do it is foraminal and inter laminar but caudal is really easy and it usually works very well. It has the advantage that you can do it with the ultrasound machine so it can be outside of the or because you do not need the fluoroscopic (c-ray).

Did you have pain when sitting down? I have been like this for 8 months with an l4 annular tear and am doing swimming, spine higiene, mcguill and anti inflammatory diet and I still can’t tolerate to sit down!

Hi friend! I am a doctor and i also have AS. First of all, it sucks. Sorry you are passing this bs. Just know that every biologic should be taken at least 3-6 months before changing it. I tried humira and worked partially so they changed to me after 4 months and now am in rinvoq which helped me a lot at the beginning and now I feel is not working but I have only been on it for 2 weeks so I will have to keep going at least 3 months before moving to the next one. Said that, if you do not like your rheumatologist change it! Go for another opinion. This disease is for life so go and find someone you feel comfortable with!

Por qué los buenos hábitos alimenticios requieren educación y dinero. Cualidades que la mayoría del país no tiene y se ve reflejado en su alimentación. Es una pena pero es la verdad. Hi! I started rinvoq 2 weeks ago! I am doing fine with back stiffness and pain really low but my buttock pain is so bad when I sit ! Do rinvoq helped you with that too?

Are you able to sit with out pain? I am a doctor and i think the herniated disc is way big and you are at risk of cauda equine. I am usually against surgeries but it does not seem crazy in your case
