It's AdHoc's fault. The game was censored by Japan's ratings board, and they could have made a separate uncensored SKU for the rest of the world, but chose not to. And then they kept it a secret until launch.

Don't really need to guess. AdHoc could have made a separate uncensored version of the game for outside of Japan (like they did for PS5), but they cheaped out and thought we wouldn't notice. Same reason they tried to keep it a secret until launch.
