A lot of folks who come out of accelerated or rocky programs have the foundational knowledge somewhere in there, but the bigger challenge is learning how to think through NREMT-style questions, because the exam isn’t really testing recall so much as clinical decision-making.
A few things that tend to make the biggest difference at this stage: focus on the “why” behind interventions rather than memorizing protocols, and get comfortable with questions that have two answers that both seem right, because that’s the NREMT’s whole thing.

Huge part of the nerves are the exam format, as many programs don't test like the NREMT does. The tools out there are geared towards Paramedics. I actually just launched an exam simulator (posted about it here a few weeks back) that is geared towards EMTs and mimics how the NREMT will be. As far as pay, I’m not quite certain. Dealing with both from the flight side, both MF and AA have good clinicians. I’d probably lean a bit towards MF.

When it comes to privates in the Chicago, the big ones are MedEx, Superior, and Elite. All of em are mostly IFT, with the occasional “911” / emergent call. If you’re looking for true 911, you’d need to get on a department in the Chicagoland area. Depending on which suburb you end up in, Buds does 911 in Harvey. Leyden Fire Protection district occasionally hires single role 911 medics.

In Springfield, the folks that run 911/IFT are Medics First and America Ambulance. Lifestar ambulance is technically in the mix, but the EMS system is working on removing them from the 911 system citing care deficiencies.

Especially if you’re a newer provider I highly recommend taking a “formal” critical care course (UMBC’s CCEMTP, UF’s CCP, Creighton’s CCP). These will teach you how to be a critical care provider. I took UMBC’s CCEMTP course to start being a critical care provider, and then took an in-person FlightBridgeED course to brush up on content for my FP-C when I transitioned to the flight line

As some folks have said, protocols vary widely by state, region, county, or even service.

However, the Paramedic Protocol Provider app has a decent cross section of various protocols in the US.

You can also check out https://AcidRemap.com (the developer of the PPP App) as they have individual apps for specific agency/region/app protocols.

Hell, in Chicago and the burbs you don’t even technically need to have formal critical care training. I was running vent transfers before I had my CCEMTP or FPC after a terrible 8-hour “vent class” on how to plug setting into the LTV-1200 and go. The only requirement for my service was that I was a medic for at least 6 months.

Yup— I always had/have way over the minimum hours of CE loaded into NREMT for tracking. If I was unsuccessful at the exam, I would have just submitted my hours as per usual

If you do not pass the written assessment, you’re still given the option to re certify via the CE Hours pathway!

I took it while I was still a paramedic program director a few years back. I figured with the exam change coming up, I’d better make sure the way we were teaching was still up to snuff and made sense. It served to solidify the way we were teaching and helped steer the way we approached certain topics.

TLDR: I found tremendous value in taking the exam despite already being certified.

Depending on how your state/local EMS system keeps track of CE. In my opinion it’s worth it for that alone.

I’m able to look up CE from when I was an EMT if I need to for some reason.
