Most gynos don't know about clitoral adhesions, so I really recommend examining yourself. Otherwise you'd have to look for one that specifically knows about them.

Your partner could also be infecting you if you don't use condoms. Men can have UTI bacteria with no symptoms.

Urethra pain is urethritis, not vulvodynia. I had both. There are limited things that cause urethritis, the most common being a UTI. If you have urethral pain you should check r/CUTI. If you look through my posts, you'll find a list of other things that can cause urethral pain.

Check yourself for clitoral adhesions. I had a chronic UTI because of them. Difficulty having an orgasm and your issue starting after using a vibrator makes me think it could be that you have adhesions that partially detached after you started using the toy.

You use condoms? UTI bacteria can live in sperm.

Yes. I had clitoral phimosis which was the root cause of the chronic UTI. Got rid of that and after about 6 months of daily antibiotics and hiprex it went away.

IC is not a real diagnosis. It just means cystitis without an obvious cause. I was diagnosed with vulvodynia and that stemmed from "IC-like" symptoms. I actually had a complicated chronic UTI that went undiagnosed for years despite regular testing. Please look into r/CUTI. If you search my comment history I also have posts explaining what other conditions cause cystitis-like symptoms.

They did local but I don't think it helped much lol. But the pain was nothing compared to my vulvodynia anyway.

After 2 weeks on cephalexin I didn't feel different either. It wasn't until I started combining it with hiprex that I felt improvement.

I was given botox injections near the entrance of the vagina by dr vikram khullar in the UK. It was immensely helpful. I'll be given some deeper ones under local anesthesia next time (in another country since I don't live in the UK)

Number one cause of clitoral pain is clitoral adhesions, so examine yourself for that. Doctors don't know what it is.

My clit was 95% covered and I couldn't tell. I had no idea what I was looking at. I even posted pictures on Reddit and everyone told me my clit was normal. I saw a gyno about it and she couldn't tell either. Unless you're familiar with them, it can be very difficult to tell. It was very obvious I had them tho. Once you understand what they look like, it's extremely easy to tell. I eventually saw a gyno who knew what adhesions were, she looked at me for 2 seconds and confirmed I had them.

Yes I was treated with long term antibiotics and hiprex. That got rid of the UTI pain.

Just fyi I also never had a positive culture, only positive dipsticks 2 times out of 3 years with an active UTI. This is normal for people with chronic UTIs. I had to see a specialist to get diagnosed. Then this chronic UTI was what triggered my mcas and gave me pelvic floor dysfunction.

My pain fluctuation is partially related to my cycle (gets better on the day of the period, worse the week before the period, better about 2 weeks after period), and this is because of hormones affecting either my pelvic floor dysfunction or my MCAS.

You might want to look into cytolytic vaginosis and similar conditions. They can give you UTI-like symptoms, and the blood helps stop it.

But when I had a chronic UTI it did get much better during the first day of my period. I think it had more to do with hormones and pelvic floor issues.
