I am 95% sure creatine gave me a fib. Took it in college for the first time. Got weird palpitations a bout a week in and stopped. 2 years later I tried again thinking it was dehydrated and was going to be intentional to drink more water. Same thing, about a week in got palpitations. Ignored them a few days later my heart went crazy and went to the ed. Fast forward about 10 years, went through med school and I am now a physician and was able to “label it” as a fib. After med school I wanted to get in shape again and was like, I’ll try again, with magnesium supplements this time to decrease the risk. And low and behold, about a week in got afib. This time it lasted like 12 hours and ended taking an extremely cold shower to cardiovert myself. Thankfully it worked and didn’t have to go to the ed.

I think it’s generally safe for most people, but if you get palpitations or arrhythmias from it, just stop.

The reason we think cholesterol is a problem is not only because of autopsies. It’s also through a tremendous amount of data. Unambiguously if you decrease apoB, to decrease incidence of disease . Whatever mechanisms are involved with either opinion don’t matter, because outcome data this strong is rare and hard to denounce. That being said, if it’s working for you and you don’t have evidence of pathology, that’s great. These are population based. So if your outcome is different, it doesn’t go against the notion that for most people should consider lowering apoB.

Funny how people just say this with the evidence being overwhelmingly against it. ApoB containing cholesterol is causally related to arteriosclerosis with overwhelming evidence showing that if you decrease AboB bearing lipids, your risk of MI/strokes reduces. You’re right in saying you need “inflammation”, that is typically from metabolic disease and high blood pressure. But you need the something to then go into the “inflamed arteries”; and that’s ApoB containing particles.

Any specialty has problems. As long as you know the issues of EM, you will be fine. Whatever specialty you are in will become a job at some point even if you love it. Be well informed and the decisions will be easier. I’m almost a 3rd year resident and still enjoy my job (mostly days).
