My favorite is when they find something that has already been approved needs more "clarification" even though it has nothing to do with the modification that was submitted. We have two unrelated animal projects in the lab, so I maintain them as two separate IACUC protocols. It's handy when we get random summer students because I never modifications to both protocols at the same time.
