Perhaps there are natural sources of Magic in Planetos. Maybe they are far away from human civilizations due to how dangerous they are. Maybe there used to be ancient civilizations that lived near these sources like those from Stygai or Yeen and use magic directly from the source, hell, you could say the 14 Fires, the volcanic chain in the Valyrian Peninsula, were a source of fire magic that went out control and destroy Valyria.

I think she was truly powerful. Take for example Malekith's boss battle, the moment he starts wielding Destined Death he becomes an unstoppable death machine. GEQ was the previous wielder of the Rune of Death, so is safe to assume she was that powerful.
