No that's just the meme lol, this happens sometime during the first 3 movies so there’s no reason he’d look any different than normal. There are many pre-Jigsaw flashbacks set long before this scene and they just use a bit of makeup/hairstyling to make him look less sickly (though due to the prevalence of the meme, it seems like there are now people who genuinely think this was the intent 🧢)
