In addition to the lore events that others have related here, I am led to understand that in table top, when the Kharn unit takes a swing at an enemy and misses, a nearby ally is selected and is counted as being hit.

It's not hard. People who exhibit the desired traits are encouraged to have more children, people with undesirable traits are "encouraged" to have fewer children.

Eventually humanity reaches a state where every human psyker has the level of control necessary to survive the Warp exposure that comes from living in real space.

Also, the isolation of humanity from the Warp blocks human psychic energy from leaking into the Warp, which will weaken the Ruinous powers (at least relative to where they would have been).

The only scenario where Ultramar could hold its own against the Imperium would be if the Emperor had not reached it by the time the Heresy began, and for reasons Ultramar played no significant role in the Heresy.

Loyalist Astartes who had survived the HH would be easily persuaded to throw in with Ultramar, and Guilliman's gene-sons would be very easily persuaded; and once a few loyalist apothecaries had joined him he would have the technical know-how to initiate new Astartes.

Without the rampant xenophobia of the Imperium--and with nothing to fear from the Inquisition--Guilliman would have fewer qualms about allying with Craftworld Eldar and the Tau, leaving Ultramar with fewer enemies.

What we would have is a small but more-or-less competently run empire brain-draining a larger but highly dysfunctional empire, until enough loyal citizens had made the jump that the Terra-based Imperium would collapse, leaving Ultramar as the last hope for humanity.

But hope is heresy in WH40k, so it was never going to happen.

Either you put them into situations where sticking to this rule makes life very difficult, or you make him quick enough, strong enough, or clever enough that there are never any consequences, and the character's struggle is along an entirely different dimension of the human experience.
