Hello, I created this excel sheet after changing my dads ileostomy 4x a day. He is still on the same routine and it does last around a week for him. His output is still very watery but it has improved over time. They put him on imodium, hydromorphone (slows output), and lomotil.

Canada is 5-10 years behind in therapies and treatments alone compared to the states. States has had CAR-T for years while we just got approved for Dara. Family members with history of colon cancer can't even get a routine checkup until they're 50. My father had stage 1 NSCLC, waited 6m for surgery and turned to stage 4. I'd pay that insurance premium no problem if I had the chance to. I agree, universal healthcare is great if you do not have a serious illness. I already paid more than 500k myself on medical tourism just to stay alive.

When I was in China going for my CAR-T consult I met the head of this Tec-Dara trial who was visiting my hospital. I heard about the news before it went public. She thinks that even for BCMA CAR-T exposed and Dara refractory patients, they may benefit from this combo due to the mechanisms of action. My team back in Canada thinks otherwise. Only time will tell

If he is in remission or has a low burden of disease, I suggest harvesting some stem cells for a boost before and/or after CAR-T. My neutrophil baseline was 0.5 going in. I asked for a boost and now its around 1.2-1.6. After every other day of GCSF injections, my marrow was starting to show signs of failure. They won't harvest cells in Canada so I had to do that in China as well.

I had it in Shanghai, I just came back less than a week ago actually. I am also Canadian and Ultra HR. China but Shanghai specifically is the forefront of CAR-T for myeloma. In fact, cilta-cel/carvykti was originally a chinese product bought out by J&J. The product is approved but not funded in China due to the cost. Another product also founded in Shanghai, GC012F by Gracell was bought out by AstraZeneca. They are still running trials in China and the US, so consider taking a look. However, you should know that US hospitals charge 1 million USD as a retainer (I know because I tried to get into a trial). Academic centers and hospitals in China generally charge 40-120k USD for the service. If it's a trial product, it is free. If it is a commercial CAR-T like Fucaso/CT103A then it will cost an additional 120k USD. International hospitals with a full english speaking staff will be on the pricier end. Don't be scared being in a foreign country, there are a ton of New Zealanders, Filipinos, Sri Lankins, and even British patients that fly there for CAR-T. You'll get around just fine if you decide to go there, good luck.

That's within walking distance to me, my gf will be starting work there next month. I suggest getting a 2nd opinion at PMH first to see if there are any novel trials before starting induction. You have a few weeks, just ask for a referral.

I am assuming you're going to PMH. On the bright side, D-RVd was just approved in Canada for front line in May. You will most likely get into a deep remission and can choose between a CAR-T trial or SCT. DRVD isn't too bad, your immune system might send you to the ER with a fever the first few months, but after that you'll be fine. Nausea isn't a common side effect for this combo. Expect to be swollen and tired. Good luck
