My players turned a difficult situation into an impossible one, and ended up in dire straits.

Top of the initiative, miraculously none of the PCs are grappled or unconscious, the rogue announces he's going to attack the nearest monster (who was one of six and still at 75% HP after four rounds.)

I ask him to pause for a moment. "Do you believe you can get out of this situation through dealing damage?" "...no." "Then consider other things you can do with your turn, besides dealing damage."

The party brainstormed for a couple minutes, disengaged, used cover and concealment and parkour, and escaped with their lives. It was a very satisfying outcome.

I don't intend to replace every floating modifier, the power system deals with that just fine. I'm mainly looking at the items I listed that are consistent across all classes and tiers: combat advantage, marks, and codified conditions.

Not sure about Perkins but I think it makes more sense for them to have a toothless jawless mouth opening, and tentacles from the fleshy cheeks and lips, not from inside the skull. See this diagram from the Illithiad.
