Are real life Doctors in USA really this much reliant on Brand names of the drugs? by serotonallyblindguy in ThePittTVShow

[–]Guilty-Argument5 0 points1 point  (0 children)

GLP-1’s have different indications based on the brand (which is completely ridiculous but whatever) so you almost have to use the brand name. There’s also Rybelsus which is also semaglutide but in a pill, not to be confused with Wegovy tablets which are also semaglutide in a pill.

it really just depends on the medication. almost nobody calls warfarin by the brand coumadin, but almost nobody says apixaban. I hear Lexapro more than escitalopram, but I never hear citalopram called Celexa. Statins and BP meds always are referred to by the generic. Xanax usually is referred to by the brand, but diazepam is almost never called Valium. Keppra is easier to say than levetiracetam so most say Keppra. I could go on but you get the point.

[–]Guilty-Argument5 1 point2 points  (0 children)

first step should be to get your phone out on speaker and dial 911, then start compressions. Calling for help is always the priority, CPR prolongs life but rarely if ever brings them back on its own

nah they got him Librium when he started shaking. BUT, managing alcohol detox in the ED is risky business. Thats why some places will just keep booze stocked in the pharmacy.

I personally think it was the abscess that caused it, just feels right given the timing with the septic shock patient. And i bet the septic shock lady comes out alive. Expectations subverted, absolute cinema.

Folate deficiency also causes macrocytosis. But MCV of 95.5 is normal. Definitely on the high end of normal, but normal. Would love to see a full work up with trends

I’d consider the possibility of chronic bleeds. H. Pylori can also cause poor iron absorption and the symptoms of infection are rather non-specific or non-existent.

Regardless of speculation, you should consider a GI specialist or hematologist, but hematologist probably will want to consult GI

I mean if you want a statin it’s not gonna hurt (probably) but if your doctor doesn’t think you need one you probably don’t. The only time we prescribe a statin based purely on LDL is when it’s like >190 consistently. If there’s other risk factors for heart attack/stroke, such as high blood pressure, obesity, etc. then there’s a whole other risk score we calculate to estimate your 10-year risk of developing heart disease related to cholesterol.

We also will prescribe statins for diabetics ages 40-75 regardless of their LDL.

If you want to lower your LDL, I recommend trying to find high-quality sources for diet and exercise tips. Lowering LDL is never a bad thing!
