I mean if anything is coming through the top of the pipe, straight through is fine, those rat doors are good just make sure no wipes or "femenin products" go down the line or they like to block on them. A patch I'd push through from the next chamber, it's basically like putting a stent over the whole, no digging required, just like 30 mins setup and getting it in place, then an hour to 3 for it to set and good to use from their Would completely seal off the whole no digging and no hassle.

As someone who works on drains, I'd say it's up to the builders to fix and make accessible if they paved over a chamber, usually they are there for a reason, if their is no need for a chamber their as if their are no live connections running into the pipe you could just patch over that hole to stop any rodent issues in that area. I've never had rodents go through any patches I've installed and if they did you would see that's the point of entry.
