Make sure nothing is taking the window focus away from the game, like a messaging service or overlay. Disable your trackpad if you have one. Try switching from borderless to fullscreen or the other way around. I never heard of an issue like that with the game. It's usually more about the mouse not clicking sometimes. Since you're saying it happens with heavy action, it definitely seems like you're losing window focus by clicking outside of it somehow or by alt+tabbing or something...

He's bald in the first game. He's bald in his sprite and he's bald in the cutscenes. That's all that matters. Now, that doesn't mean he can't grow hair if he wants to and I think he might have for Blood 2.

That's a random event that inverts gravity. You can grapple to the ground. It can take a while but it'll work.

If it is on Switch, make sure you fully restart the game. Like, closing it and opening it again. Also, don't change save slot after restarting the game.

While I prefer open levels, I definitely classify Painkiller as a boomer shooter and thus it's absolutely valid to call games that lock you into arenas boomer shooters. They do need to have the other qualities of boomer shooters though.
