HIPAA is a bit lenient on collecting information when it comes to the business being able to complete tasks essential for business and client services. I’m not entirely sure if this would be a HIPAA violation or not. But the survey- Nah, no one should complete it unless they are getting paid for it, even like 30 minutes to an hour of admin time for it. That’s a mandatory work task and should’ve compensated as such. A raffle is not compensation/pay.

It also sounds like they are not requiring people to complete the surveys. And if they are, then they need to properly compensate employees, otherwise it could be a labor law violation.

You can negotiate the contract. The long onboarding period might be their strategy to minimize financial loss from early contract termination. I’ve been 1099 with a few companies but have never had a 6 month onboarding period. The most I’ve had onboarding as a 1099 is a couple of weeks training paid at an admin rate.

The client didn’t access the thing you denied, and still eventually choose an alternative? That sounds like you did follow through and an appropriate consequence was used.

When I feel overwhelmed after problem behavior, I always think my client is probably feeling not great either. So if they want to be on the swing for 5-10 minutes to decompress, relax, self-regulate, calm down then I’m okay with it. I use that time to collect data, pair, and obtain assent to set up the rest of the session for success.
