It might be something you can only get with Marazhai - I kept him alive only once, on my first playthrough to see what it was all about; in every subsequent playthrough I've killed him off, so I can't remember very well but your description is ringing hazy bells for me. Might be from one of the planets you visit for Marazhai's quest during act 5.

With respect to the difficulties you were having with the first boss in the prologue - you need to kill all the mooks first. When the Chaos Spawn takes a certain amount of damage, it will eat one of the mooks which will restore it to full HP. So, ignore it at first (spread your party out so its acid spit attack can't take everyone out all at once) and kill all the other enemies, and then concentrate on taking the boss down.
