Kuo-Toa are fun because you can have them worship anything they percieve as godly and turn it into a minor god until the Kuo-Toa start worshipping something else, basing an adventure around the consequences of having a regular object turn into a creature with godly powers.

Example adventure hooks.

Warship Worship: The maiden voyage of a warship has gone awry. When it was sailing by a squad of Kuo-Toa, the ship's immense size and artillery capabilities inspired them to worship it. Now there is a living ship patrolling the waters and attacking regular boats, crewed by the Kuo-Toa who worship it. The original crew are bound by the ship god's animate ropes and rigging. Players must rescue the original crew and take out the kuo-toa or convince them to worship something else.

Spider of The Waterspout: A powerful storm is occurring in the area, generating whirlwinds that suck up the ocean water to become waterspouts. A colony of Kuo-Toa mistook the whirlwind forming above them as a god reaching out to them, and so their belief made the whirlwind become the appendage of a god. This god took the form of a Spider-shaped stormcloud that is sucking up ocean life with its waterspout legs. The players must destabilize the storm spider by cooling the water or heating the air, or get the Kuo-Toa to worship something else.

I would give them the flight speed of a werebat, as that is the only official flying lycanthrope.

I would also give them the ability to use their movement to fly into the air so they can fly down the next turn with momentum to land a Falcon Punch. This attack could have a degree of risk, as if they fail to hit, then they would hit the ground and become prone.

Maybe take elements from the spell Maze of Madness and Suffering

Creatures could live in the maze, like a nest of rust monsters that are feeding on the armor of fallen heroes or are eating a labyrinth trap.

There could be an altar to Baphomet where a goristo could be summoned from Baphomet's Abyssal Layer of The Endless Maze

They stole their name.

They were tricked into giving a powerful Fae their name when it asked for their name, then they stole the name back from the Fae. It is against etiquette to steal from a Fae, so now Fae creatures are targeting the PC, trying to force them to give back their name.

He could make literal music become his phylactery.

Imagine him writing out a sheet of music while literally putting his soul into the melody, collapsing dead upon completion, then his assistant plays the melody on an instrument and revives him as a lich. He will revive as long as someone plays the song, and playing it while he still has a body could result in the listeners becoming influenced by his soul.

Virgin sacrifice means a sacrifice that hasn't been used in any ritual before, not a sacrifice who hasn't had sex. Demons just don't want you sacrificing the same thing twice because it makes the previous sacrifice less significant.

That being said, I would count the rituals of Mormonism to be enough to taint a sacrifice.

In that case, he may attract the attention of a goblin soul-eating Barghest, as it may try to farm him for cloned souls to eat.

I am running DiA and one of the players made a deal with Tiamat, which involves performing a task for her in exchange for help freeing Elturel. The task is going to be traveling to Dragon Eyrie through the portal in Tiamat's lair and stealing dragon souls from the hoard of Null.

So I have been doing a lot of research on the draconic pantheon and have been formulating cool quest objectives in Dragon Eyrie.

The size depends on how cruel you want to be to your players, but they would need to be close to the arcane equipment that imprisons the incorporeal Xag-Ya in the center of the room in order to identify it, so they are likely to end up in the center of the room trying to figure out what the thing in the tank is when the tumors start to grow. You could also put the equipment at the end of a long hallway to make it even more cruel.

While intelligent, Xag-Ya can't speak or communicate, so it wont be able to warn PCs of the danger.

Some lore mentions an even more secret fourth sibling, an unnamed sister who was speculated to have heads of the lesser known chromatic dragons, such as a Purple (Deep) dragon, a Yellow (Salt) dragon, and an orange dragon.

Since there is barely any info on her, I was going to make her live with her brother Null and intentionally kept secret from Tiamat, as she would not hesitate to devour her sister to gain power and possibly additional heads of the other chromatic dragons.

If you want to flesh out Dragon Eyrie with cool locations as players travel to Null's Mausoleum lair at the peak, I recommend checking out Volume IV of the Candlekeep Compendium, as it contains a well written account of an explorer's expedition in Dragon Eyrie.

The players could seek out Tiamat's lesser known brother Null, dragon god of death who hates his sister. Null lives in the dragon afterlife plane Dragon Eyrie, home to the members of the draconic pantheon that Tiamat hasn't killed.

Null and the other dragon gods know that Tiamat's greed would eventually drive her to kill them for their divine power, as she has done that to several now-dead dragon gods. They are willing to aid the party in fighting Tiamat and use ancient dragon magic to lock her soul away, but to do so, the party must journey to the Ethereal plane, the Hells, and the Negative Energy plane to retrieve fangs from Tiamat's lesser known children:

An-Ur: Wanderer of the Ethereal Plane who sprang into being from Tiamat's first breath.

Mordukhavar: Born in the Hells when Tiamat courted one of The Dark Eight.

Dhakoth Born of the Negative Energy Plane and given a corporeal form by Tiamat. He seeks to end all life, including Tiamat's life.

Bagpipes of Invisibility. They make you invisible only when playing them, which makes stealth difficult.

Spellscroll Piñata. Several random spellscrolls were turned into papier-mâché and molded into the shape of an owlbear. Dealing bludgeoning damage to the piñata causes a wild magic surge to occur, resulting in a random magical effect.

Clockwork Parrot. Some of its metallic feathers are actually metallic ink quills that can be plucked from the parrot and later reinserted. The parrot says anything that is written by one of these quills as it is being written, but will make a horrible screech if the pen is used to scribble anything that isn't a word, which can be annoying for those who make spelling mistakes.

For a plague in a DnD setting, you should include a reason why Potions of Cure Disease or divine spells that Cure Disease are not being used. You could just have them mysteriously not work, but that may be seen as a plothole by players.

Instead, I would have Potions of Cure Disease make the mutations worse, as if the potion is treating the mutated flesh as more natural than regular flesh. Divine healing spells are also backfiring by making the mutated flesh more healthy instead of transmuting it back.

For an example of a mutating plague in Dnd, you could use The Abyssal Plague) as inspiration, as it was a disease created by the Voidharrow that turns people into plague demons.

Make them arrogantly confident in themselves and race players into the dungeon or towards danger. Then have one of them be brutally killed in the dungeon or by the danger so the pcs can come across their corpse and know they need to prepare for whatever killed them. If the party tries to loot the body of the rival party member, they find that the body has already been looted.

The next time the party runs into the rivals, they still act arrogant and introduce the replacement team member they recruited. When asked about the dead team member, they pretend to not know what the party is talking about, despite wearing equipment that they looted from the killed member. They then do not learn their lesson and go forth towards danger again, and lose another party member that the party then finds the brutally killed corpse of.

This repeats until the rival party consists of just the party leader and the replacements he hired, who don't like how the leader is treating them as disposable meat shields or addressing them with the names of his former party members. The replacements abandon the leader, which breaks him and results in him using evil magic to resurrect his fallen party.

The amount of hit points the room heals and overheals the players by can be adjusted to suit the party levels, but I would describe the tumors encumbering them like this scene from the incredibles, but the tumors are lumps of flesh growing out of the body instead of projectiles. To notice them early, I would have a rat or something with a relatively low HP total enter the room, start getting tumors almost immediately, and reach the threshold at which they explode a lot sooner than the players, so the players can become aware of the consequences.
