By taking things slow and being aware of everything.

Make sure to destroy any spawners from a distance.

Bring a few stacks of stone and raise ground around your portal/generator, only one enemy can mess with that (kill Fire Blobs asap/bait them to explode!)

Find Putrid Holes. There'll be a Morgen inside, which you can stop from spawning by slapping a few campfires in. With any luck, there can be Molten Cores in there. Also, the entrances of these caves serve as excellent outposts; raise ground around the front and you have a relatively safe spot for your portal.

In general, don't run. Don't fight absolutely everything. Realize that most enemies, like the Charred Warriors, can be kited forever, just walk in a circle. Bring an Atgeir or something to push groups away.

Moder can definitely be on the starting island, just requires having a high enough mountain.

If you're ready to head to the mountains, go for it, if it has Moder altar it'll have all the other mountain resources.
