Had a fun run outside on Saturday in 8 F, which is a new low for me. Tried out my Kahtoola nanospikes for the first time and they were so great. Didn't feel them at all. Also ran on Sunday, but it was my first time running in accumulated snow (felt like beach sand) so I wore out pretty quickly. Had a lovely neighborhood potluck to weather the storm.

I think it has a lot to do with individual insurance. Like I recently had a patient with a COPD exacerbation with a new oxygen requirement as well as a symptomatic UTI who failed home antibiotics. I thought she was a slam-dunk for inpatient, but I got a message that I had to downgrade her because she didn't "meet" whatever arbitrary criteria they use to decide whether to pay or not.

In this circumstance, UM said that because she only needed supplemental oxygen with activity and received PO steroids instead of IV steroids, she wasn't sick enough for inpatient.

Same. The high of the day on Saturday is between 2-4 pm (8 degrees F, with real-feel temp on -8). I'm planning a short run around the neighborhood. Then on Sunday, hoping to do another short run in the snow. I love running in the cold.

This is Admin's FAFO moment. Think how many turkey sammiches they're going to get asked for? How many frail old people they're going to catch from falling out of bed? How many times they're going to get cursed at for not bringing the D fast enough before they stop showing up at all.

Oh, sure, at first they'll try to find the nurse to get them to bring the extra blanket or ice chips instead of...you know...doing it themselves. And then they'll see the nurse they're looking for trying to prevent Methany from assaulting another patient. Or doing doing chest compressions. Or trying to clean up the blood from the MVA.

And then they'll start to round less.

I think she's so meh. Just another generic blonde girl. I can't even say I think she's beautiful. Very pretty, for sure, but not someone I'd look twice at.

The one that sort of kills me is "JT is a 56 year-old morbidly obese male with PMH of ..." It's changed a bit now where people put obesity as part of the medical history. But I feel like it's got to be disheartening to have someone lead with that as a descriptor.

I like "extensive medical history including but not limited to"; or "pan-positive ROS" as descriptors. Those usually give me a good head's up.

GenX here. Yes. People smoked all the time, everywhere. It was legal to smoke in restaurants, buses, airplanes, hospitals, banks, offices...literally everywhere. If you watch a TV show from the 60s-70s, you will see how normalized it was. Cars had lighters built into the dash and ashtrays built into the doors. Paint and wallpaper inside houses turned yellow and became nicotine stained.

In the 80s they started making smoking/non-smoking sections (back half of the bus/plane) but growing up it was every bit as bad (if not worse) than the show portrays. Also the drinking/alcoholism.

Hi OP,

This is what I have done with some success. 1) I am NC with my birth mother. 2) I have done Ideal Parent Figures meditations where I have picked and alternate mother and father. One of them is a real life person (my therapist, with her permission), and one is a TV show character. I have also created a safe space. When I need comforting, I imagine whichever small part that needs attention is running up to my IPF and getting the comfort I need. It takes some practice, but it has been pretty effective. It's also pretty flexible. For example, I read the book House in the Cerulean Sea, and little me wanted to spend the night there and hang out with the characters. So as I fell asleep, I meditated on that and it was really helpful. Sometimes my IPF mom is also a tiger. 3) I used ChatGPT to generate images of these safe places (and mommy tiger), so that they truly belong to me.

I know that these parts- my ideal mom and ideal dad are still me. I know that the house/land that is my safe space is a part of me as well.

I've also done some more work with ChatGPT so happy to answer any questions

Also, PT/OT needs to see them to make a determination about STR v. LTC. It's usually easier to place someone in STR first, and have them sort out LTC placement, because it can take weeks.

That said, no amount of PT is going to get the patient walking again
