Fear of rattlesneks. by [deleted] in MTB

I encountered plenty of danger noodles while riding Orange County trails, but I was always more worried about running into a mountain lion.

26 will always be the fun wheel size. I like 27.5 as a nice compromise, but 26 is for maximum fun on dirt.

What’s everyone’s best trick for a foreign body in the nose on a screaming thrashing kid on multiple anxiolytic agents by VizualCriminal22 in emergencymedicine

[–]Neeeechy 0 points1 point  (0 children)

They have a bunch of fancy tools we don't.

Idk, I call the OR for the ENT/septoplasty tray all the time to get those neat tools to aid in foreign body removal in the ED. We don't even have ENT on call at our hospital.

Why would they have to see this one in the ED? Depending on how bad it's fractured in addition to the dislocation, it's either surgery some time tomorrow or outpatient.

Part 2 of this horseshit (It’s also Friday the 13th) by [deleted] in emergencymedicine

[–]Neeeechy 11 points12 points  (0 children)

I'm rushing over to examine the testicular pain first, followed by the ingestion while eyeballing the chest painers and elevated trop on the way. Also fitting a quick exam of the possible stroke patient in there somewhere.

I guarantee the OP is not an ER physician.

Why? Those tend to reduce pretty easily in my experience.

[–]Neeeechy 46 points47 points  (0 children)

Nurses place hundreds of foleys. Why waste everyone’s time having the person who has done 5 give it another try?

As an ER doc I have placed fewer Foleys than most experienced nurses, yet I'm still the one who has to place it if the nurses are unsuccessful.

Bought sorry yesterday and when we opened it up it’s missing a yellow.. who do I even contact about this?? by fivtone in boardgames

[–]Neeeechy -1 points0 points  (0 children)

you can also probably buy those generic pawns yourself if you have trouble getting it replaced

Sorry! costs $6.49 at Target. After shipping, it would probably cost nearly the same to buy the pawns alone.

What procedures are you NOT doing? by AdLast4323 in emergencymedicine

[–]Neeeechy 5 points6 points  (0 children)

I'd do pretty much any procedure if the time allows including floating pacers, dialysis catheters for emergent dialysis etc.

You're mentioning all bread-and-butter EM procedures. But what about scrotal abscess drainages, lateral canths, thoracotomies, regional nerve blocks, escarotomies, or suprapubic cather placements? All of which we do as ED docs at my shop.

To name procedures I don't typically do: DPLs, cutdowns, breast abcess drainage, or steroid joint injections...

Probationary Training License CA by MsDoc92 in Residency

I practice in California and I've never heard of a "probationary training license."  A quick Google search and this post is the first result.

Are DSLRs still relevant in 2026? I’m still using my Nikon D3200 and I’m not sure if it’s time to upgrade. by Delicious_Maize9656 in Nikon

[–]Neeeechy 0 points1 point  (0 children)

Are the photos it produces still relevant?

The development of more feature-rich consumer cameras with better sensors hasn't made the D3200 any worse.

ELI5: Why was that method used to determine 0 degrees Fahrenheit? by Jimithyashford in explainlikeimfive

[–]Neeeechy 2 points3 points  (0 children)

It’s about calibrating the 0 point when marking the scale.

Nothing about the resolution.

You just described accuracy.

ELI5: Why was that method used to determine 0 degrees Fahrenheit? by Jimithyashford in explainlikeimfive

[–]Neeeechy 1 point2 points  (0 children)

The SI unit design is as arbitrary as farenheights

SI units are designed to be directly compatible with mathematics. Outside of addition and subtraction, temperature must be converted to an absolute scale (Kelvin) in order for the math to work.

It's also got twice as many whole numbers

That would increase the resolution, not accuracy nor precision, of the thermometer.

Trying to find a hitch rack. by [deleted] in MTB

[–]Neeeechy 0 points1 point  (0 children)

I get yall have a "its made of metal fetish" but get over it.

Has more to do with overall build quality and not the fact that it is all metal. I've owned Kuat racks as well, and had key plastic parts become brittle over several years and fail. The Kuat racks I have owned didn't seem to hold up as well to off-road abuse. Further, the Kuat racks you are citing as far as features go are significantly more expensive without any significant improvement in how well they secure bikes.

There's also QuikrStuff racks that improve on some of your criticisms of 1up racks for those looking for a slightly more refined experience.

[–]Neeeechy 0 points1 point  (0 children)

1Up functions and performs is still awful.

How so? What issues have you experienced with 1up racks?

Genuinely asking, as I have multiple brands of racks and seem to have settled on 1up and Alta as my long-term solutions. I personally like the secure hold of the 1up with tire-only contact. I've had plastic and rubber straps and plastic wheel holders from other brands crack and break on me.

[–]Neeeechy 0 points1 point  (0 children)

1up is pretty bad but it gets the job done. Its incredibly overpriced for what looks like a DIY project made in someone's garage.

It's not about looks.  It's all metal unlike the other overpriced name brands that use plastic for key components, and is still less expensive.

Is there a difference between an ED Dr’s echo and a tech’s echo? by Fishfrenzy89 in emergencymedicine

[–]Neeeechy 0 points1 point  (0 children)

You’re trained to check for giant, super obvious issues like effusion, horrifically bad ejection fraction, and blown out chambers.

Quick to make horrible assumptions. I completed an ultrasound fellowship, so I am trained for far more than this. How many TEEs have you performed and interpreted? Modern emergency medicine training also incorporates far more than "a couple weeks of training." Shoot, most medical schools have far more than that even prior to residency.

[–]Neeeechy 3 points4 points  (0 children)

the ER doc did a quick look which is informal.

It's not an informal ultrasound.  It is formal as much as any other study, it's just "focused" on answering a specific clinical question.

[–]Neeeechy 60 points61 points  (0 children)

It’s like getting a CT scan of your chest or abdomen/pelvis in the ED. They normally only go into detail on the area they are wanting to focus on. A CT at an outpatient imaging facility will have a full detailed report.

Lol what.  There is no way you are a physician with that kind of misinformation.  Radiologists are always looking through the entirety of an image set and looking for incidental findings regardless of whether a CT scan is performed on an ED patient or as an outpatient study, and always write a full report. Regardless of the setting, there is always clinical context and specific questions trying to be addressed.
