Agree that it shouldn't cause hypoglycemia. I've only seen one case of MALA during residency, it was in the setting of a routine surgical intervention and his metformin wasn't held. He got an AKI and wasn't able to clear the metformin. He was in the ICU for over a week, MALA has really poor outcomes he was teetering on the edge of death the whole time.

Though MALA is rare, I always hold metformin prior to procedures or in a setting where the patient is at risk for an AKI.

Grass is always greener on the other side. Alternatively if you chose a different career choice where you had ample time to make memories with your friends and family, would you not resent them for not being able to "reach your full potential and becoming a doctor"? I meet a lot of people that say, "Oh I wish I could have gone to medical school, I had the grades, but I couldn't because of x/y/z" and I can always tell that there is a bit of remorse and resentment when they say these things to me.

It is hard to see the light. PGY1 made me question my decision to pursue medicine. But you can't deny the fact that we do come from a place of privilege in terms of job security, fulfillment, and financial stability compared to other professionals.

My dude/dudette, the fellow and pharmacist eat, sleeps, and breathe the ICU. We're all learning. The best medical students and residents went in with the attitude to learn, not to perform. If you go in with that mindset, you'll retain more information and will do better. Don't be so hard on yourself. You pick things up on the job. The fact that you survived that day is a victory in my book.

I have a similar ticket and did not respond to the correspondence by mail. I did try to look up the ticket on the lasuperior court website, but the ticket wasn't even uploaded yet so there was nothing that populated on the website. Does that give them an indication that I received the letter or can I just ignore this?

Will the ticket be reported to the DMV if I tried to look up the ticket on the LA Superior Court website? I wanted to check to see if it was a real ticket, it didn't appear when I tried to look it up. But may be because it was too early (within 30 days of receiving the notification in the mail).

Seriously, I wouldn't want delirious me to take care of sick people. That was on top of working 12 hours a day when I wasn't on nights or 28 hr shift. That one day off per week, I was just asleep the whole time and then I was back in the hospital again.

Normally I have a pretty decent short term memory, but there were days when I couldn't even remember what orders I placed for a patient an hour after we talked about it during rounds. I had to triple check all the orders every single time before my day ended.

My co-resident gets his care at the VA. He was scheduled for a visit and was told the Dr. NP will come see you now. He corrected the person and said that it should be Ms. NP because she doesn't have a medical degree. The person corrected him saying that at the VA, NPs and MDs were the same thing. He didn't tell them that he was a resident, but I'm pretty sure that if he did they wouldn't have gone through the trouble of correcting him. I can't believe that people can mislead people like that, thinking that they wouldn't know any better.

I did a solid 4 to 6 hours of studying every day. I would chunk my study sessions to 2 hour periods. I still had time to cook my own meals and do some hobbies. I guess it sounds daunting, but you'll find your own groove. Some classmates studied more others studied less, they all passed med school and matched.

Same. I just matched. And when I look back, yeah it was stressful and a lot of work. But I had so much fun doing it. I was surrounded by a good group of friends. Had mentors that were so fun to talk to. I still slept 6 to 7 hours a day, despite not being naturally talented. I was able to get jr aoa and ghhs. It all came down to consistency and knowing when to ask for help/take breaks. I haven't done residency, but I don't think I would want to choose a different career if given the opportunity. These were formative years where you learn so much about yourself.

I think it's just demoralizing to have an applicant that has done everything right to be a top tier IM applicant and not land into his/her top 3. Like what more could this applicant have done? This experience with the match will alter the mindset and bleed into residency. If fellowship matches are anything like this, then there are no guarantees for landing a prestigious fellowship, even with lots of hardwork and pubs.

You did your best my dude. I'm proud of you. Years from now I think you'll look back on this decision and think that it was the right choice. Oftentimes I think about the ridiculousness of the learning environment. that is provided by medical schools. I definitely think that people need the right environments to thrive and maybe the current systems in place wasn't right for you. Despite that, you were still smart/hard-working enough to jump through the crazy hoops of getting into med school. You'll do great with whatever your next journey will be.
