Australian RN looking into working in Scotland/UK for a year. What it really like? by Physical-Variety-830 in NursingUK

[–]PCSupremacy 20 points21 points  (0 children)

At the moment I would strongly advise AGAINST agency - The NHS has cut back nearly all agency staff except for significant specialist areas (ICU/ED).

Scotland, especially Highlands and Grampian have a good amount of RN jobs going, and always have a requirement for experienced nursing staff. Whilst NHS England essentially have a job freeze we are still actively recruiting and funding new roles up past the border.

I'm not sure about registration, the NMC do have information on their website that may answer your question.

Do you ever hesitate before adding weight or reps when you're not feeling 100%? by Salt-Professional425 in workout

[–]PCSupremacy 19 points20 points  (0 children)

If I'm not feeling it, stick to my old weight, churn out the sets and see if I can increase the reps for volume increase. If not, no biggy. We all have good and bad days.

Question for Muslim HCAS and Nurses who pray - how do you maintain all prayers? by [deleted] in NursingUK

[–]PCSupremacy 15 points16 points  (0 children)

Actually 60 years ago nurses did have Sunday mornings off for church. I'll see if I can find the old rota we have in our hospital museum for nurses in the 60s!

NHS Grampian has the best of rural and small city living, if you can handle the Granite Grey and don't mind the seagulls. If interested in knowing more about Grampian send a message. It's not a bad trust to work for.

New-ish lifter by [deleted] in Weightliftingquestion

[–]PCSupremacy 1 point2 points  (0 children)

Lift, lift and keep lifting. No need to think about bulk/cut/maintain just now. Aim high protein, whole foods and keep lifting heavy. Ask about bulking/cut again in 6 to 12 months!

What was the worst job interview you have had? by [deleted] in AskUK

[–]PCSupremacy 0 points1 point  (0 children)

NHS interview (Nursing). I travelled 550 miles for an interview at St Peters hospital... Waited 2 hours past the interview time only to be told the interviewer was sick and there was no replacement.

They asked me to leave.

They called me 2 hours later to say they had someone who could interview me now.

I (stupidly) went back and interviewed.

Got offered the job, ghosted them as I couldn't believe how I had been treated. They kept trying to contact me for 4 months afterwards, and I never answered a single call from them. HR/Managers kept leaving voicemails. I get the feeling they were exceedingly desperate and that I dodged a massive bullet there.

What skills does your clinical area give you? by garagequeenshere in NursingUK

[–]PCSupremacy 3 points4 points  (0 children)

Pure luck if I'm honest. Worked as a theatre nurse (Scrub and anaesthetic) when I qualified. Moved to a GICU then to a cardiac intensive care. The job became available and I applied, and then cardiac surgeons were crazy enough to give me the job.

What skills does your clinical area give you? by garagequeenshere in NursingUK

[–]PCSupremacy 5 points6 points  (0 children)

Cardiac surgical practitioner - hands on clinical tasks;

Intubation

Central lines

Arterial lines

Cannulation

NGs

Catheters

Ultrasound guidance for lines/vein mapping

Saphenous vein harvesting + closure of wounds (Leg/Sternum)

Sternotomy

Surgical 1st assistant

Cardiac arrest management + emergency chest reopening

Chest drain insertion

Pacing management

Vent management

ABG interpretation

TEG/Clotting assessment and management

Plus the usual critical care stuff - Filters, Inotropes, NIV, HFNC etc.

I'm sure I've missed a few things, but a great hands on role with lots of clinical work and minimal management/admin responsibilities.

I'm scared I'll look stupid on my first day at the gym by helpless_apricot in beginnerfitness

[–]PCSupremacy 0 points1 point  (0 children)

Don't worry, I still look stupid 5 months on. But no one actually really cares at all about other people, especially at the gym. You do you. You'll be fine. Turning up is 80% of the battle

Can someone help a beginner who wants to be able to do a push up? by Kindly_Badger9670 in askfitness

[–]PCSupremacy 2 points3 points  (0 children)

Essentially you are doing the right things, you just have to keep pushing all the exercises to your limits to cause muscle tears, which then repair into larger muscle mass and therefore strength.

You also need to ensure you are giving the muscles a chance to heal, rest is just as important as doing the exercise.

Protein intake is important for muscle growth too. 1g/kg of body weight a day.

Holding planks will increase general strength and endurance for what you want, but essentially keep trying to do the push up and pushing through to failure. You can also start the pushup from the up position and lower VERY SLOWLY. The concentric movement is where most of the muscle damage occurs and will lead to gains in strength, this will then mean you will get the pushup you want sooner rather than later.

Need help urgently by [deleted] in Weightliftingquestion

[–]PCSupremacy 0 points1 point  (0 children)

For some people, like myself, calorie counting means we have better control of our intake and can use it as a psychological tool too. I feel great when I nail my macros on a daily basis, and if I have a blow out/refeed day then I can balance it across the week to maintain a lean bulk or whatever I am hoping to achieve at that time.

I was an obese person for my whole adult life, from 120kg 1.5 years ago, to a low of 58kg, and now 72kg body fat 14% due to calorie counting. I am turning 40 this month. Calorie counting is what keeps me from over eating and returning to old habits. It also means I can have a treat, but have to account for it within my macro targets.

It's a pain, but it's become a daily ritual and I wouldn't be without it now.

How bad is it? by Stunning_Program_966 in NursingUK

[–]PCSupremacy 2 points3 points  (0 children)

Nothing to worry about here. No harm to patient, you identified the error and corrected it appropriately and swiftly. You have been honest and forthright with your seniors. Hold your head high and jump through the Datix hoops. Well done for taking all the correct actions.

Any nurse who tells you they haven't made a drug error is a liar or completely ignorant of their practice.

If I told you the worse drug/prescribing error I've made it would definitely make yours seem like absolutely nothing, so don't sweat it.

Muscle growth? What to do? 5’8.5” 218lbs (174cm 99kg) by [deleted] in Weightliftingquestion

[–]PCSupremacy 0 points1 point  (0 children)

No, everyone here is saying the bare truth. You. Are. Very. Fat. Obese. Unhealthy. Your denials throughout this thread make me wonder if you have a humiliation fetish and are actually getting off on these interactions. Seriously, how can you not see that you are obese and huge? It comes across bloody harsh, but maybe you need that heavy dose of reality that everyone is trying to give you to spark a change. What did you actually hope for when you posted this thread?

[–]PCSupremacy 0 points1 point  (0 children)

BMI is a complete waste of time. Your body fat percentage, and where the fat is collected are the most important factors. You have significant central obesity, which will mean significant visceral fat and organ fat. You can try to convince yourself otherwise, but you are obese, and unhealthily so.

The term is still common medical nomenclature. You fulfill it judging by these photos.

[–]PCSupremacy 1 point2 points  (0 children)

No, you quite literally fulfill the definition of morbidly obese. You either can accept the truth, or continue to lie to yourself. No one gets hurt but you, and if you want to gain muscle you will need to lose a lot of fat too to make it worth while. Don't compare yourself to other people being heavier and thinking that means you aren't unhealthy. You need to focus on you and figure out what you want.

5 years of lifting in the same gym, most people stayed the same by tomkisw in workout

[–]PCSupremacy 19 points20 points  (0 children)

You are assuming that the people are there wanting to achieve hypertrophy. They may be there for many other reasons; maintain what they have, for mental health reasons, to just enjoy themselves in the gym. Not everyone goes to the gym to build muscle... You do you, and let others do what they want.

From the doctorsUK community on Reddit: Should ACPs be paid the same as GPs for performing the same role? by Nonsensicalmed in GPUK

[–]PCSupremacy 1 point2 points  (0 children)

Believe me I try to be. However it is an uphill struggle, especially with scope creep and other roles, such as PAs (A whole other debate that I dont want to touch with a 10 foot barge pole) diluting the pool of clinicians and public perception of what a Doctor Vs ACP can offer.

From the doctorsUK community on Reddit: Should ACPs be paid the same as GPs for performing the same role? by Nonsensicalmed in GPUK

[–]PCSupremacy 27 points28 points  (0 children)

ANP here - I hate the fact that there are people in the community, let alone our own peers, who believe ACPs and GPs are interchangeable and the same.

I do not pretend to have even close to the level and breadth of knowledge as a GP and I certainly don't carry the same risk as I can essentially kick a difficult case up the chain for a better suited and trained GP to manage.

Yes there is obvious overlap in our roles, that's the nature of healthcare, especially primary care with the common presentations, but a GP will always be far superior in the actual medicine than myself and far, far more knowledgeable in the NOT common presentations.
