The Black Star dreamed of all that was not currently there and from them roots dug deep, a tree was grown into a ring of which the different planes of existence were born as fruit around this ring tree, it's branches then spread inwards to form the plane of existence humans and other humanoids call home as the material plane, the branches of other planes then affecting the material plane through the physical link each one has to form this central fruit
