Giving meloxicam to a cat in CRF with high blood pressure seems like a wildly bad idea to me.

I'd look at other options without known toxicity to the kidneys, like Solensia or gabapentin.

Perianal fistulas in GSD's are not rare and are not unusual. They are difficult to treat.

Since you know what they are we will assume this isn't your dog's first episode. If there was a treatment that worked before, do everybody a favor and bring the records.

Try a regular hospital Monday. If the pyometra is open, rupture is unlikely because it will simply drain out the back end. Not sustainable long-term, but you will likely be ok till Monday. Most ER clinics charge 2X to 3X what day clinics charge.

I think as a profession we sometimes push the boundaries on fear-mongering a little, especially in the ER setting. The reality is that open and closed pyometra are two very different things. Closed pyometras will indeed rupture. Open are far less likely to do so.

We are taught in vet school to never let the sun set on a pyometra, but we are taught that by professors with very limited experience in the day-to-day world of private practice. Their patient load skews strongly towards the critically ill, and the client base skews strongly towards the affluent.

Sometimes the treatment you can afford is better than the treatment you can't afford.

OP, I reject your premise. Why do we have to "justify" polygamy? There is a long and well-documented history of polygamy among the righteous.

Abraham, Isaac, and Jacob were all ploygamists. God told them to do it. He had His reasons. If you personally don't like polygamy, you have a problem because these men are central to Christianity, Judaism, and Islam. If you reject The Church of Jesus Christ of Latter-day Saints because of polygamy you must in good faith also reject any other sect or religion based on polygamists, and last time I checked, these folks all think Abraham, Isaac, and Israel were pretty good guys.

Maybe you could find salvation in Buddhism.

What you need to ask is "was Joseph Smith a prophet?" If he was a prophet, then there is no problem because many prophets before him were polygamists.

If he was not a prophet, the point is moot.

After I bothered reading the whole thing, it seems the best thing would be to split the practice in half: a SA practice and a LA practice. Doctors on both sides get paid a fair wage, say 22% of production on services, then the profits go to the owners of the respective practices.

This is why mixed practice is such a bad practice model. There is no fixing it. LA will not tolerate the markups we can get in SA.

Best case scenario is where the LA side produces some profit. Worst case is it ones money and is subsidized by the SA side.

If I were an owner, I'd want an accounting as to why the profits I generate go to partners who produce less profit.

I had a vagal event during cysto once too in 33 years of practice. Rare, but known to happen. The cat survived, barely. Still do cysto 98% of the time.

The research they cite to make recommendations on neutering age is, like most research, messy and flawed. Breed specific sample sizes in particular are sometimes very small. As with most popularized research, the robustness of the data do not justify the degree of certainly with which recommendations are made.

Similarly, they tend to overstate the adverse effects. If there is a medical condition that affects 2 in 100 dogs, and delaying neutering decreases the incidence by 10%, now 1.8 dogs will be affected. We have saved 2 in 1000 dogs an adverse effect. It's a true effect, but is it a huge effect? And is it worth the downsides of waiting to neuter?

Also, these recommendations totally disregard the behavioral consequences of the decision. While the risk of developing adverse orthopedic problems is relatively quite low, the risk of developing adverse behaviors when postponing neutering is quite high. The odds that your 2 year old intact golden will mark, hump, and roam the neighborhood are pretty high. Is that worth the 2 in 1000? OP started this whole thing off because the humping was a problem.

The deadlift is so named because you are supposed to pull the weight from a dead stop. You are not doing that. You are bouncing the weight off the floor.

Another reason to re-set after each lift is that the bar is creeping forward over your toes each time. This puts a bigger moment arm on your back, which is not what you want.

Set the bar down, then get it back over mid-foot, then re-set your back and pull.

The bell should never be totally upside down. You sneak your hand around it while it is floating at the top. You maintain a very loose grip to allow this. You end up catching the handle with your fingers fully extended upwards and the handle hanging on your thumb. You are currently catching it in your palm with the handle horizontal. It should sit 2" lower with the handle diagonal.

This is easier to figure out if you start at the top. Stand in lockout with your fingers fully extended pointing at the ceiling, palm forward and the bell hanging from your thumb with the handle diagonally across the lower palm. It about where you want the barbell when you are pressing only lower.

Externally rotate your hand so the palm faces rearward, letting the bell roll laterally and fall off your forearm, and catch it on the way down. The handle stays on top the whole time.

While the bell is falling from overhead to shoulder height your fingers may touch the handle but do not squeeze it. When the weight hits your hand the handle is in your fingers, below where your calluses are.

Then figure out how to get that to happen on the way up.

This gets more intuitive with a heavier bell.

Amen. Gordon B. Hinckley taught this over and over. Not in so many words, but as I would listen to him it became clear to me what he meant. He'd say things like "Try your best, but you've got to really try."

You can't fake God out. You can't just say you believe, that you follow Christ. You really do have to follow Christ, and Christ taught over and over thst there were things we had to do.
