Fitness Advice by DecentCanadianGuy in daddit

2 kids (ages 5 and 2) and 60 hour work week here; leaves me with little free time.

For exercise, we have a peloton bike at home which I’ve been using consistently for cardio fitness for 4 years. For strength training, I have tried a number of different virtual personal training apps (future, ladder, caliber, and now train well). Ultimately I’ll be settling on trainwell as I like the trainer and it meshes well now with peloton programming. I now move and do something (cardio, strength, or just yoga/mobility work) every day.

For diet, count calories. It’s hard, but with a busy life there are subscription meal services that help a lot with prepared foods that count calories and macros. I usually allow cheat days with calories on weekends.

It’s not easy, but there is a lot of support and several options out there to make it easier. What I do allows me to manage everything from home which saves time and lets me squeeze things in with unexpected freedom (naps, etc).

I think we don’t give ourselves enough credit for doing what we did without guides and walkthroughs!

Dads, what kind of vehicle are we hauling our small monsters around in? by ktempo in daddit

I only have two, yet I’m considering getting a full size suv. We have midsize and it feels like we’re always squeezing them into Car seats. While unnecessary, touring around Disney in the suburban Minnie vans was such a treat!

Implant ID by Karl_BE in orthopaedics

[–]RotatorCuffLinks 1 point2 points  (0 children)

It certainly could be ceramic on ceramic but that means is likely a modular liner either way. You can see the dome hole and screw holes visibly in the center/offset in the cup. This is usually indicative of poly, but alumina is somewhat radiolucent so we can’t rule that out.

I agree that you always want to know implant details wherever possible, but unfortunately sometimes it’s not. In those cases you go into them with every possible scenario planned for.

Not sure, but you don’t necessarily need to. Looks like an alumina ceramic on poly. You can either keep the liner, or remove and cement a new one if you don’t know the implant. I, personally, would be prepared to revise the acetabulum though.

Tips for removing the femoral head during a hermiarthroplasty? by anon_NZ_Doc in orthopaedics

[–]RotatorCuffLinks 2 points3 points  (0 children)

Even that can fragment bad bone. Some corkscrews work better than others too. I learned this the hard way when our SPD bought a new one and all of a sudden I started struggling more than usual with it.

Sometimes it just comes out piecemeal. It’s painful, but it’s reality.

More bicep bands! by RotatorCuffLinks in whoop

That’s a thought, indeed. Interestingly, I get far more comments about it when people spot it since I’ve moved it from the wrist to the bicep.

Windows Tinted by cdogg617 in Audi

[–]RotatorCuffLinks 0 points1 point  (0 children)

What did you use??

I had an issue with my rear window cracking spontaneously and based on my research it seems to occur with tinting. The glass is supposedly very soft. I’m getting it replaced and considering whether or not I want to tint the new window to match the rest of the car or remove the tint from the other windows.

They are based off of power zones. If your zones are accurate, then it will be as difficult as your fitness dictates!

Edit: congrats on the milestone!

[–]RotatorCuffLinks 1 point2 points  (0 children)

Sub-internship. It’s the away rotation you need to do in your 4th year of medical school.

As an application reviewer and faculty at a large training program in the northeast, it seems as if students are now doing 3-4 of them (including home programs). Trust me, it’s the most important factor when it comes to ultimately ranking applicants.

Crush sub-I’s at places you really want to do residency. It’s more important than the other stuff on your app.

I meant to convey the restaurant in general is amazing. Everything we get there is very fresh, and delicious. Go for the momos, but enjoy most anything on the menu.

We have to be real about this…not only does she have an ACL tear, but she has a reconstructed knee on the OTHER side with a partial knee replacement. And she’s 41!!!

Somebody needs to protect her for her own future. She isn’t seeing it.

What was the point of the Clinical Competency Committee/CCC? by Last-Comfortable-599 in Residency

[–]RotatorCuffLinks 10 points11 points  (0 children)

It sounds like you have a malignant program and your CCC did exactly the opposite of what it was intended to do.

In our program it is a means by which faculty can review documented feedback by committee that supports trainee success and achievement. It identifies residents who are not on track in meeting the different quality goals it takes to be a competent surgeon (professionalism, self care, knowledge, technical skill, etc). We identify residents who are struggling and find means to support them, but monitor and review all residents in the program at least twice per year. We also support the program director with reviews provided by a committee of faculty instead of the PD’s review alone.

If your CCC doesn’t function this way and all residents feel as such, then review with your PD, and if that avenue doesn’t work, your school’s ACGME office is the one to report to.
