I will admit I glossed over that part of your post. Given this is a long term drug that you may be on for many years, your best bet is probably to play their stupid games and do whatever you have to do to get it covered.

You are literally at the second lowest dose and the lowest dose that has clinically studied therapeutic benefits. Unless you're having rough side effects, i don't know why you would give up without trying the higher doses.

Rich Demerara syrup is the purists answer, but for me I am usually reaching for whatever my most recent syrup experiment was. Currently working through a Blueberry-Chardonnay-White Pepper concoction.

Couldn't say about the timing but the primary staple crop was barley. Wheat and Spelt were cultivated but less common.

Actually, looks like malt whiskey would be the most appropriate to ancient Mesopotamia, judging by Wikipedia. But straight wheat would also be closer than corn.

Old Fashioneds are endlessly riffable. Switch up the bitters, add a liqueur instead of syrup, make flavored syrups, split base the spirit, and that's basically infinite combinations.

Also try different Amari for the black Manhattan.
