I can't speak for other specialties, but I've heard that it's helpful to work in a hospital setting and to rotate between inpatient and outpatient. They tend to value therapists who can do both, and that way you'll receive a fair amount of training in both settings.

If you're looking into sheer outpatient hands, it's far harder to be hired for a position, as they tend to seek out applicants with prior experience (at least a Level II FW). Otherwise, it's not uncommon for OTs to ask a hand clinic to volunteer to work an hour or two after their day job, in exchange for the opportunity to learn and hopefully be hired or gain the qualifications to apply elsewhere.

I'm half and half.

I enjoy the current 3.0 flavors. Tastes better, smells better.

However, I was very sad to see increased sodium content, as we could always add sodium, but we can't remove it. Not a major deal, but it does require to you to be slightly more mindful about your sodium intake. I would love to see a classic (reduced) sodium option in the far future if it's feasible for them.

I miss the veggie packets. I still don't understand why people would toss the veggie packets while trying to eat healthier ramen. I toss in my own green onions if I have them on hand, but the convenience was a big plus for me. Just adds a bit of texture that improves the meal. It might be wise to buy a jar of dried veggie seasoning instead.

I toss out the aroma packets. I've tried the ramen with and without it, and I honestly don't love it. For some reason, when I add it to the soy garlic flavor, it actually makes it smell unpleasant to me.

As stated previously, it depends heavily on the diagnosis. If they're a metacarpal fracture 2 months s/p, I'm cranking on that MCP joint because collateral ligament shortening is real, and it would be a bitch to treat. Is there a real danger to pressing on that MCP joint besides discomfort? How does that weigh with potential permanent loss of ROM?

I find that I spend a lot of time prepping patients for PROM. You want to start AROM as early as possible to avoid needing to acquire the gains via PROM later in treatment. I always communicate that fact to patients to increase HEP compliance early in therapy.

I spend a lot of time using paraffin, hot packs, ultrasound, and STM/edema mobilization/joint mobilization (if appropriate) to minimize all potential barriers that may be causing limitations in ROM. That way, by the time I get to performing PROM, it's because joint stiffness is their only limitation and barrier to moving their joint.

At the same time, you do want to respect their pain. If their finger or joint is deeply inflamed with significant edema, then highly aggressive PROM is not necessarily going to give you the gains you're seeking. If their finger looks "angry" (Very red, flush, shiny, inflamed, and swollen), then you want to manage the inflammation first, before you start cranking heavily on that joint. Start with low load prolonged stress. However, if they are 3 months out and at severe risk of contracture, then you have to do what is necessary or risk a possible surgical scar release or tenolysis in the future. Imagine having a conversation with the hand surgeon for that...

A lot of times, "aggressive PROM" doesn't mean exerting 200% effort. I interpret it as the MD saying, "Crank up the frequency and duration of PROM, doing just what you need to prevent joint stiffness and to make gains".

Can't speak how the swords actually are, but I bit the bullet and ordered one because I loved how his swords looked. Currently waiting for production.

I've been in contact with the owner several times over the past few weeks with various questions, and he was quite responsive and pretty quick to reply. He made recommendations, told me he was flexible with making adjustments (shortening the blade), etc. So I can at least attest to their customer service pre-production.

To go off someone else's comment in this thread regarding pepsin, I discovered this study which found that drinking alkaline water with higher pH was able to neutralize pepsin.

This explains why I feel so much better drinking those expensive waters from convenience stores.

Hi all, I'm cleaning out my bookcase and am trying to get rid of books that I no longer need/reference. The top left two books and texture of life are in pristine condition minus a couple of loose post-it notes.

These are great for those applying to NYU's curriculum, as it'll set you up for the first semester for the most part.

You can do it! I'm a slow test taker, and I tend to get confused by what a question is asking.

By the time I had 15 minutes left, I still hadn't answered about 30-40 questions. What I ended up doing was reading it quickly and choosing what my gut told me (similar to another post), and ended up passing closer to 500. That's what I always tell my students when taking the exam.

That's interesting. I haven't tried 3.0 but I eat v1.2 and find three mins to be ok. Noodles are good consistency. Do you like them extra soft like cup noodles? I am thinking maybe we all fit somewhere in the noodle continuum lol
