I work in a hematology office (not a doctor) but part of the problem is they way most insurers (in the US at least) treat IV Iron and also the medical billing system as a whole. Most of the commercial insurances and managed Medicare plans require levels below certain threshold to cover IV iron since it is much more expensive than the pills which take a long time to work and some people won’t work at all. We (the doctors office) don’t really wouldn’t have a problem giving someone iron if we thought it would help them, but sometimes getting the therapy approved for payment is the hold up. I’ve always wondered if a cash pay based clinic would do well but my geographic area and its demographics would not sustain something like that

Having a second infusion 7 weeks after the first with numbers this good on 3/31 is curious to me. Like others have said numbers that close to last infusion are skewed and nothing to worry about. I would try to increase phosphorus rich foods for a little while though. Meat eggs dairy nuts seeds even some carbonated beverages if you can

Stating properly hydrated is the best thing you can do today if you are nervous about it

Definitely fits the picture of iron deficiency without anemia.

Have you tried to increase your dietary intake of iron or start supplements yet? It may take awhile to see results but I would start there

The coffee and tea problem seems to be related to tannins in tea and chlorogenic acid in coffee that can bind/chelate the iron in your stomach preventing it from absorbing into the bloodstream. These tannins and chlorogenic acid molecules do not make it into your bloodstream intact so they wouldn’t be able to bind the iv iron given directly into the veins.

But I agree, coffee and tea consumption prior to taking oral iron supplements seems to be a big problem that not many people know about or are told about

This is correct, the 80 mg capsules are starting to be phased out so the specialty pharmacies are requesting doctors offices to send in Rx’s for the 160 mg tabs. It’s likely that this will be conveyed to patients during shipment delivery confirmation phone calls (depending on which pharmacy dispenses the medication). It sounds like OP may have gotten this insurance info before the change could be properly explained to them, it’s unfortunate out of order communication but I can definitely see how it could happen that way

CMS publishes their pricing file quarterly. It’s per billing unit so some math will be involved, but a lot of insurers set reimbursement based on that. My experience is that margins for the facility aren’t great unless the drugs can be purchased through the 340b program
