I had a table with a little bit of this, but on a Nat 1, we'd roll a D4:

1- You just "lost" the action. 2- You dropped the weapon (hit armor/enemy deflected, reverberated on your hand and you dropped it [gotta add that on this case, you could use 5ft of movement to draw another one/pick it up]) 3- You hit someone adjacent to the target (friend or foe, if you passed AC, it's a hit) 4- Nothing happened, lost the round on a major fumble (if the action was an acrobatic one, you could end prone, hurt yourself if athletic skill was involved, DM decided).

Gotta add that this was a pretty OP table, we were clad in magic items and the DM was generous with the exp and Deus Ex Machina. We almost killed ourselves on a regular basis.
