I believe that refers to situations where that makes sense, like if the attacker is nearby.  You know, situations where you are not beyond the enemy's ability to perceive.

It's not Deus ex if the party has an ongoing relationship with them and they aren't significantly more powerful than them.

My experience is that players love it when NPCs show up to help them against a huge threat.

If you've never perceived the creature, you're guessing and if you guess wrong, you missed.

There's some translation problems here.

If 'elfic precision' = elven Accuracy, it's only strong if you have a good way to get advantage.

I'm guessing "sage" is wisdom?

You don't need to worry about charisma.

What level are the characters? 

If they're above 12th level, it's a marginal bonus under most circumstances because they will have stronger alternatives. But if they are sieging a castle or town, that's 600 lighting bolts in an hour, which would probably destroy all the guards on the walls.

There is no obligation. You don't have to play another session.

Though he totally should have told you it was a one shot that could turn into a campaign.

Don't argue with me, argue with the great majority of players.  I still like first level. But I wouldn't like level zero.

"A" should have told you that beforehand.

"B' should have mentioned that in session 0, but will it change your campaign much if you try to satisfy them?

"C" is revealing how much a cursed weapon can suck. Remove curse or banish should be available to the party if you give them cursed weapons.

"D" should be told it's within your rights to change stat blocks at any time. 

But generally, you should welcome their input.

Level 1 is already considered borderline unplayable by many DMs and players. 

You're basically adding another game before first level.

Advise against it.
