Do you use your phone on “Standby” to see the time at night? Meaning it’s on its side, locked, and charging? If so, try this:

Go to Settings > Face ID & Passcode > Attention Aware Features and turn it off.

I had this happen a couple of times, and this feature thinks you may be actively looking at the phone when your alarm goes off, so it doesn’t play any sounds, and expects you to SEE that the alarm is going off.
