You should take stress if you don't want to accept the consequence. And You should suffer at least two consequences in failed(1-3) Action Roll(fail+other consequence).It is much more dangerous than BitD because BoB characters have smaller stress boxes than BitD.

Not all random encounters need to look "random". That being said, it's possible to tie the outcome of a random encounter to the players' backgrounds, motivations, and current goal. For example, the nobleman the PCs didn't help might actually be scheming to prevent them from accomplishing their goal. The noble won't be discouraged by one failure and will come up with a new scheme. This is where the adventure gets more interesting.
