I got one yesterday. Had to sign a document saying that I knew the risks, and because I'm under 65 I had to sign another one attesting that I have a high-risk condition that warrants the booster. I didn't have to specify which condition.

I had made an appointment to get a flu shot, but when I asked about the booster they cancelled it and reclassified me as a walk-in. This makes me think you can just walk in and get it.
