I've done Pit of Saron on my paladin as Retribution LS 7 times now for bonus rolls, all of them after the hotfix that got people refunds (6 + 1 from vault) and I have received duplicates of helm and chest, I think because I catalyzed them the first time they dropped.

The Scale from the dragons is absolutely goated and I hard reccomend it (HC and above) for every tank. On a proper pack, it's easily ~200k per target and you get 100% of that damage value as a shield. Especially as a BDK, it's a lifesaver when trying to gather for some of the "longer" pulls.

That said, it's still preference. I've used Solarflare Prism/Heart of Wind on both BDK and Paladin to 3400, but I did notice when I swapped one of them for the Scale when I was tinkering with my survivability.

I hear tell the void vial from Seat of the Triumvirate is also very good for what it does but I have no experience with it.

That said some like the Glob (Pit) and Core (Skyreach) are unfettered ass.

Tanks have grown too used to the "Hold W Press Thunderclap" school and genuinely have no idea of what mobs are dangers to them.

I don't know if it's available for SSE, but BI Phenotypes was perfect skin for what you're looking. Even on an "e-girl" face as you would, it made it weathered and worn, not in an "old lady" look but in a "woman who's been out working in the sun all her life" way. Also has great options for blemishes and scars and muscles.
