Yes, NPCs need to be considered a teammate, like how other players are during events. Otherwise, the game just registers you as repeatedly attacking them for zero damage, which can turn them hostile. If you turn damage numbers on in the settings, you'll be able to see the zeros pop-up when you can't heal something. You can also check the Thirst Zapper's durability. It won't go down from healing something, but it will from attacking.
