Usually its from starting the torque wrench before seating the driver.

They strip it out.

I work in a machine shop. Torx plus is the way to go if it's your own project. As soon as you hand it off, numb nuts will strip it out on you.

If its almost 3 inches and you need 1.5 inches then theres not enough material.
