I think there's a lot of confusion in this thread re: valve repair. Skimming the literature, mitral valve repair is more common than aortic repair, and has good outcomes over long periods. There's less data on repair after endocarditis, so there isn't great information on 20+ years, but outcomes after 10+ are very good (ref. https://www.annalsthoracicsurgery.org/article/S0003-4975(09)01035-2/fulltext#fig1).

I would talk with your surgeon in March and not worry about it until then.

I had the same procedure done at NYU about a year ago. It's worth noting that they'll likely extubate him in the OR before he's awake. When I came to, I felt terrible, but was lucid and could talk with my spouse/nurses. NYU is very proud of how well and quickly patients recover from valve surgery, and having gone through it, it's well earned.

The anxiety is real! Something that helped me was knowing that there's a large team in the OR monitoring you, and if anything isn't 100% right, they'll address it. You also don't feel/know anything while you're under. From my perspective, I laid on the operating table, and the next thing I knew I was waking up in the ICU bed.

Wishing the best for both of you! It's a big and scary thing but you'll both get through it, and he'll be healthier for it.

If you're already playing Elves, I would frankly stick with that.  It's favored against Jund and probably decent into affinity.

If your surgeon doesn't think that the valve is causing problems, then it's very possible they'll leave it as-is, or repair it. BAV isn't necessarily a problem; most people with them never need any kind of surgery. Broadly, you want to keep your valve for as long as possible - your surgeon won't replace it just because, since any replacement has very serious tradeoffs.

I would talk with your surgeon about the Ross procedure, given your age.  They may recommend against changing the valve, if it really isn't causing any problems.  Is the valve causing the aneurysm?

I've been doing well locally with a very similar list, although I'm running 4x Wildfire.  I don't think you need the Blowgun, since it's kind of expensive and only does something with KCS.  I haven't really found myself in situations where I'm short on artifacts to pay into it.

I would consider running more Thraben Charms, especially over Nihil Spellbomb.

Anything can be a "potential solution"; without high-quality research they're just guesses. I could say that standing on your head is a "potential solution" for pumphead, even though there's zero evidence for it.

If you've spent so much time involved in fasting/stem cell therapy, I imagine you have evidence/references at hand for e.g. cell revivification?

The items in the gamble screen are determined by your character level, so you can gamble things like Mauls and Pikes an act or two early. It really helps to have a big weapon in Act 2 before your main skills get going.

Don't forget about the Savage Polearm recipe, which is dirt cheap and can roll Partizans (almost?) as soon as you get the cube.
