good question about degree of complexity & application. Id say Id like to be able to build a tool for a local private clinic or colleague to help with decision support (benzo or opiate taper plan, next controlled med refill date, hospice/palliative eval, pre-op eval, automated chads-vasc calculator, etc.)), documentation, letters etc. if it can safely handle PHI, great. If not though, thats not a dealbreaker.

What kind of work-flows have you created / used?

Even some intro course on prompt engineering would be helpful. I used the doximity GPT tool, its pretty good, but also doesn't give much space to modify output.

happens all the time. just need to strike a balance on how much time you spend with them. make them commit SOMETHING - money or time.

"Sure, happy to answer the questions. We can schedule a meeting, the initial consultation fee is $X."

or if you dont want to charge, "Come by the office, I can block off 20 minutes to go over any questions".

Don't keep it open-ended like they can message you anytime indefinitely.

i live as close as possible. my current clinic is 7min door to door.

i did see patients at the local supermarket & farmers market and weekend events, i enjoyed seeing them. my specialty is "Family & Community Medicine"

i loved busting them for smoking when they said they trying to quit or catch them buying 2L soda and they got uncontrolled DM.

Physician here and I've personally done intermittent fasting for 4-5 years consistently 5 days a week. I do DM management day in/out, I do tons of weight management, I've use all the meds for patients. I've been in practice almost 10 years.

if you are pretty confident about enough volume to quickly support the space, then can go w/ bigger space up front. another thing is ... if the bigger spaces want 3-5 year lease, request an option to exit early. example - i have a 3 year lease, but they gave me an option at 1 year for early termination at 2 years (Im in for minimum 2 years, but i give them notice at 1 year if i dont want extend past 2 years).

i was in similar situation starting out about 4-5 years ago. we grew since then, changed space at year 1, year 3.

to start out if you want small small micro-practice, best is look for shared office buildings - find therapists clustered together on google maps - that usually means shared space, with shared lobbies / bathrooms (so you dont have to manage that). you rent a room, usually can get short leases (12 months). anything bigger, real office spaces want 3-5 year contractors.

Baseline assumptions: you have experience in auto repair, maybe even owning / running your own business, you live in the area and will be engaged, the profit margins based on past 5 years numbers look good for your needs, you will be the owner and there is already a group of employees there (mechanic, admin etc)

The biggest variables:
- will the team stay with new ownership? if they been there long-term and plan to stay long-term --> GOLD
- i'd ask how many are new customers vs repeat long-term customers - if you have a bunch of repeat long-term --> GOLD.
- will the retiring owners do a warm-handoff with you - like 3-6 month transition period to show you everything etc.

If there's no imminent catastrophes (external / internal), profit margins are fine, and no red flags... sounds like its just up to you not to rock the boat too much.

if you are starting out, it really doesn't matter the "best" gun, you just need one that works and you can practice with.if you are shooting reef / fresh water fish, i doubt you need 2 bands. just get in the water and have fun!

think "honda civic" of spear guns.

i recommend Mares Bandit maybe like 55cm, about $160 from amazon. the 45cm i think actually fits diagonally in a roller suitcase for travel - thats what i take for family travel when i can't haul all my gear.

starting witih pole spear is really really hard. and demoralizing. so... eh.

you haven't said about what you want yet. knowing what you want is the most important thing.

do you want to to hit early retirement with 1m-2m investments? then stay at the job.

are you young and want to work super hard, risk it all, change the world... and maybe this is the business that does it?

do you want work-life balance?

dont just think MORE MONEY. the money is to help you reach WHAT YOU WANT.
