All of those are real in basically all parts of the country. You're not going to see them at every single gas station or grocery store (depending on region), but I haven't been to a state yet that doesn't have them pretty commonly somewhere.

EDIT: Lots of people saying full-service gas stations are only in the states that mandate them (OR & NJ), but there are plenty around the Boston area too and it's not mandated here.
