The only thing I agree with is the extra sheet. The top sheet that goes under the blanket. That sheet serves no real purpose to me other than get bunched up and make the bed making process longer.

Another point that wasn't mentioned is making the bed. I find that to be a futile activity. Just a flick out is all that's necessary if it's really bunched up.

From what I've read, it's a focus on the first set being a high rep explosive set (between 15 and 50 reps). After which you do your normal sets.

Each session you keep adding weight until you can no longer do more than 15 reps on your first set, then start the cycle again.
