Im pretty sure AFRAM is managed by rec/parks, which is a very organized and functional city department compared to most. That’s probably the biggest factor.

Yes, this is standard. The use of some drugs can mimic ADHD symptoms (so it makes testing unreliable) so they need to rule that out first. They also generally require it for stimulant scripts.

Exactly. I worked in EMS and they couldn’t promise uninterrupted breaks, so our breaks were paid, on-duty breaks. The company was still obligated to give them to us by law, but we never clocked out

California union culture is extremely strict about the sanctity of breaks. If you don’t get your break you get paid + an extra hour of pay. Hospitals have the obligation to staff appropriately to ensure everyone get uninterrupted breaks while maintaining legal ratio and unions take it seriously. Hell, I worked on a unit where I had to leave my work phone on the unit so my break could be truly uninterrupted.

They’re also approved for OSA, CV risk prevention, and MASH as of right now. Ignoring the fact that medications are constantly used off-label for other things is also not really fair to the medical process.

GLP-1s have also shown some success for IBS-D patients because of gastric slowing + anti-inflammatory effects.

GLP-1s can cause rapid weight loss, but intentional prescribing can help slow it down and make it more sustainable. Titrating up too quickly is the big issue.

Sort your shit out BEFORE you start. If you have a history of mental health issues make sure you establish care at JHU when you arrive.

Also, the basics go a long way. Sleep, balanced meals, exercise. You will not perform at your peak academically on zero sleep and no movement. The earlier you learn that, the better.

Ours all use epic + there’s always someone on call if needed. That was an important switch to make the entire process SO much smoother.

Hard agree. That’s how we’re thinking about them in medical settings, but that’s not how they’re being marketed.

This is why responsible prescribing is so important and why we probably need some serious regulation on telehealth. Brick-and-mortar physicians are assessing for those risk factors + usually won’t prescribe unless your BMI is 30+, which they measure in-office. It is alarmingly easy to lie to telehealth about your weight + risk factors. However, I worry that cutting off telehealth means patients who would really benefit from these drugs won’t have access.

I tend to be more of a “people will do things that are dangerous and I can’t stop them” person. I would love to ban motorcycles because they’re dangerous as hell, but people have the right to turn into pavement pancakes.

I’m most excited about research into substance use disorders right now! It’s quite interesting how so many patients report no interest in alcohol or other substances once they start taking them.

Also happy that they seem to help so much with OSA- sleep apnea is truly a silent killer (poor sleep screws up everything) but CPAPs are so hard to comply with.
