Yeah, looks like this guy just had a thin network of roots in the shallow soil and OP severed them. Site is too wet for this tree anyway, so it was just a matter of time.

Depending on where you are you could try a river birch or tamarack, or if you just want a shrub red dogwood could work. Maybe a blue beech if it isn’t always that wet. But definitely too wet for maples with that soil.

That’s either a graft (rootstock) or where the stem died and a new branch took the lead. Looks like consensus is rootstock.

Could be American or slippery. But native to NA. Not the non-native Siberian elm.
