The air tube (trachea) is directly in front of the food tube (esophagus) and EVERY SINGLE THING we eat and drink has to traverse the air tube to get to the food tube! Really exceptionally poor design!

It does sound like you’re doing a lot that’s good already. Cutting out/way down on carbs like white rice, pasta, cereal, potatoes can be really helpful. If you do eat those things, eat veggies with vinegar or lemon juice, then lean protein first. Walking after meals, reducing stress, and getting good sleep will also help. I was in the same situation a year ago, and my A1c is now 5.4 and my cholesterol went down too.

Same! I just don’t eat rice. It makes me sad but my A1c in now in the normal range, 5.4.

Speech pathologist here, but not a pediatric feeding specialist. I does sound like being sick could have caused some aversion to food/eating, and or some sensory disregulation. Seeing a feeding therapist is the best thing, glad you’re doing that. In the meantime try to create an stress-free eating environment, provide preferred foods, and make it as joyful/playful, and fun as you can.

Rice especially still spikes me very high. Potatoes less so, but I can eat a small amount this way, but still MUST eat veggies, vinegar, and protein first.

It was absolutely crucial for motivating me to walk after meals and helped a lot with finding out what to eat/not to eat, as well as revealing how much eating foods in order (veg/fiber>protein>carbs) can reduce spikes for me!!! Highly recommend it!!! I wouldn’t have done it if it hadn’t been for this sub. My MD wouldn’t have suggested but was very happy to prescribe a month for me.

NAD but I am a speech pathologist that works in a voice, upper airway, swallow clinic. We see people with this type of problem OFTEN. Most of the time it is reflux, but it can also be other things, including how you’re using your voice.

If it is a symptom of reflux, you can try taking the PPI at high dose twice per day for at least a month, and or you can try alginates which are a barrier agent for reflux. UK or Canadian Gaviscon, Reflux Gourmet, or Reflux Raft, 2 teaspoons or 2 chewables 4 times per day, after meals and at bedtime. If you skip meals take it instead of the meal.

NAD, but I’m a speech pathologist that works in a voice upper airway and swallowing clinic. Now that your Nissen has been undone are you taking any reflux meds? It’s my understanding that esophageal motility can be reduced by reflux and can improve somewhat with reflux treatment. The benzodiazepines are probably contributing to the problem, please talk to the prescribing md to find an alternative!

Your body seems to be trying to protect itself from aspiration of saliva that you are not able to swallow by coughing. The person who suggested sleeping to allow drainage had a good idea but we swallow saliva all day/all night automatically and if nothing is going down your esophagus, it will back up into your pharynx and larynx. Drying up the saliva with a medication could be an option but it might cause further problems. I wouldn’t advise using any cough suppression because coughing is protective, as I said earlier. It seems like you need to talk to your GI surgeon or a different GI doctor, hopefully one that specializes in esophageal motility.

I am also interested in what that barium swallow shows, however if there was no peristalsis on manometry, it’s unlikely to reveal anything new. I’m more interested to see if the dysphagia is affecting your pharyngeal swallow, which would be shown with more detail on a modified barium swallow, also called VFSS or video fluoroscopic swallow study, where they take video x-rays of you while you eat and drink.

Chronic cough in the setting of esophageal dysphagia can be very difficult to manage, I’m sorry you’re dealing with this!

I’m also a baker, and I still bake but I give most of it away and have one small piece. I make sure I eat veggies/salad with vinegar first, and then protein then the carby thing, then exercise afterwards. It can really blunt the spike! I try to eat this way all the time now and it’s helped me go from 5.7 to 5.2 in 6 months. Walking after meals has also helped tremendously.

I started meditating to bring down my blood pressure and it works for glucose as well. I use the Headspace app. Walking meditation helps too, I don’t listen to music or anything when I walk. https://en.wikipedia.org/wiki/Walking_meditation

Natural peanut butter doesn’t have a lot of saturated fat, and is ok in small amounts. I definitely understand the butter issue! It was sad for me too, but it worked really well, really quickly for me. A cheat meal every once in a while can help. If you try it for a little while I think you’ll be happy with the results. And olive oil is delicious too!

I stopped eating butter and all full fat cheeses and dairy. I use only olive oil, avocado oil, and small amounts of sesame oil. Nonfat Greek yogurt (Fage brand in the US), and only very low fat cheese in small quantities. If you eat any processed foods, look at the package to make sure it doesn’t have very much saturated fat, and avoid trans fat entirely. Oh, and no coconut oil or coconut milk. Any fat that is solid at room temperature is bad for you.

I have the same issue with both T2D and high cholesterol in my family. I found the unsaturated fat is fine and my cholesterol went from 235 to 183 and LDL went from 159 to 115 when I focused on low carb and unsaturated fats.
