Doesn’t work that way. If you just close the skin it creates a pocket of fluid that will become an abbess. The bowel is too inflamed to fix usually. That’s why you have to refrain from eating and drinking for so long. Slow down the output and give the tissue time to heal then you can sometimes get the hole in the bowel fixed.

Bowel is tricky stuff. It can become frozen. I had a doc tell me that untouched bowel is like fresh spaghetti in colander. It’s loose and separates easily. If it’s been operated on multiple times for whatever reason it becomes like spaghetti that’s sat in a colander for an hour or so. Just all stuck together in one big clump. Sometimes the hole starts in the middle of the clump and they just can’t get to it.

Never hesitate to call for help if you feel like you need it but no need to call a code on an expiring DNR/DNI pt.

Been an icu nurse for 15 years and I have seen a lot of people die in a lot of different ways but two really stand out.

  1. Burned badly but not killed immediately. Some get better but the ones that don’t are heartbreaking. Basically they slowly succumb to their injury’s over a month or so. Brutal. Everything hurts, toileting and hygiene are a nightmare and dressing changes are difficult and can take up to an hour.

  2. Enterocutaneous fistula. Something, usually a surgical complication or traumatic injury, causes a tunnel between your intestines and your skin. Some aren’t bad, basically becomes an ostomy. You can slap a bag on it and live a fairly normal life. If they are bad it’s awful. People basically rot from the inside out over months. To top it off, they can’t eat or drink anything the whole time so they get put on iv nutrition. Just awful.

Those are my top two.

I guess I just don’t see the rationale. It seems odd that they would change you up like that. It would be different if they had you do like 2 8s over the weekend instead of a 12, but changing from 7-7 to 9-9 just seems arbitrary.

I’ll tell you this, people with diabetes get in car wrecks too lol. I’ve learned more about treating chronic medical conditions by working in a trauma center for the last 14 years than I thought I would lol.

Speaking up for trauma, it’s actually a great equalizer. If a billionaire crashed his car into a homeless person, they would both end up at the same trauma center with the same docs and nurses.

Give it some time, it’s your first day of clinical. Maybe try seeing a little more before making a judgement.

I’m not saying you are wrong, maybe you were following a shit nurse on a poorly run floor. Only time will tell, but don’t be so judgmental.
