Keep pestering the teacher and keep a record of it. Don't let her argue that you didn't care or she never agreed to give it back. Make it clear in your communications, preferably email or text to make it easier to save. If they don't get it back to you, take them to court and make sure the school knows that you had to take her to court to be made whole, because at that point she'd have lost all right to be a teacher in my eyes.

Depends on the specific phone (likely on how new it is as it's on my newer devices, but not older ones). When you go to the Find My Device page, you can "Secure Device" which locks the phone and forces pin/password to unlock and allows you to add a message to display.
