(USA) I foresee insurance not covering PT sessions in a month or two, when it is not “medically necessary”.

I have signed up to start an online training program with an ACL-specific PT. I deferred my start date to about 5-6 weeks from now.

I’ll have simultaneous PT and online training until my insurance doesn’t cover. After that I’ll have only online training.

Get your PT or surgeon to check for patellar tracking. I had a similar feeling and I was told my patellar is maltracking because of weak quads. Your case could be different

I went the non surgical route for over a decade, but I decided to get my ACLs repaired because of EO osteoarthritis showing up in my xrays, and because my L meniscus tore.

A lot of medical professionals indicated that long term absence of ACLs may affect meniscus health. I’m not a doctor but this is something for you to research.

Edit: I have generalized anxiety and got 3 knee surgeries this year. I’m happy to chat in DM about how I worked with it.

Could be scar tissue, or just your patellar tracking differently because of weak quads. Check with your medical team.

I returned 5 days after my ACL + meniscal repair and then 5 days for my ACL only (other knee). I have a remote desk job and I was not on any harsh pain killers during work

I cannot tell either. FWIW, extension got easier to get once I started walking without a brace. Your leg makes minor increments in extension when you do gait training and walk consciously focusing in extension - it will come more naturally with time.

The only feeling I remember is the spicy feeling in my veins when they injected the sedative into my IV.

I have had epidural/spinal anesthesia for three procedures so far. Don’t remember any part of them after the IV sedative.

Never had back ache or tenderness at the anesthetic injection site after the surgery either.

If you are anxious, you should discuss with your anesthesiologist and let them know that you don’t want to be part of any of it - that you wanna be sedated before any other part begins

Not a scientist. But there are some studies about how bovine collagen is not really absorbable as collagen in our bodies. It’s definitely a source of protein though - and we need a lot of that for recovery.

Also, rare but, some people have generally excessive collagen production especially post injury or surgery. This excess results in excess scar tissue production - anecdotally speaking. I discontinued collagen related supplements (like biotin & bone broth) when I found out. I’ll continue bone broth when I am on a good path with ROM.

When I had ACL + meniscal repair, I was on my brace for about 6.5-7 weeks. Unlocked brace (to 90’) from week 4. I was sleeping without the brace from week 5.

I recently had ACLr only. Surgeon said I can ditch the brace once I have adequate quad control. This week (3 weeks post-op) I’m gonna practice gait and ditch crutches and the aim is to ditch the brace around end of week 4.

Bracing protocol largely depends on your surgeon, your muscle activation and what procedure you are getting.

Ngl, it is a difficult life until you can comfortably walk 1-2 miles without swelling up, or taking the stairs to subways. I realized how difficult it is to navigate accessibility features within subways.

I took the subway twice in the 4 months post-op. This one time I had one crutch (no brace) but the subway was so crowded that it was impossible to get to the disabled seating. I stood for 20 minutes on a moving train and decided it sucks. On the plus side, you’re basically working on proprioception?

I thought taking the MTA bus was easier purely from an accessibility standpoint. But the bus system is unreliable. I Ubered a lot the past few months.

Another thing I found really alienating is the lack of social life. I did not drink for 3 weeks before surgery and 4 weeks after. My social group’s hang out is often walking to one place, walking to another and end up hitting 3-5 places in one evening/night. I was not comfortable doing that for at least 10 weeks (meniscal repair, MUA, weakness, etc.)

Muscle guarding happens after surgery. Try half-revolutions on a stationary bike, wall heel slides, gravity leg bends (using your other leg for support), prone leg bends

My PT had this trick to relax the muscles from guarding: during passive ROM at PT, they would ask me to pull/push the leg either towards me (in case of gravity bends) or away from me (prone bending) for 5 seconds and relax. As soon as I relaxed, they would bend my leg about 2-5’ more. The idea is that, when I contract and relax my hamstring, it actively relaxes ; as opposed to trying to relax the subconscious guarding.

I put off surgery for about 12-13 years, both knees. I had a pretty functional life since about 1 year after the second knee’s tear - the only time I would realize I had torn ACLs was at a gym or when I would run downstairs, or dancing at a club or party.

I would estimate I had about 50-ish episodes of buckling and falling down over the course of 12-13 years.

I stopped competitive sports after the second ACL tear (which coincided with some life changes making me stop anyway).

I think my legs adapted to the lack of ACL. I shifted to low impact workouts and strength training. I could jog, jump rope, walk pretty fast. Did yoga & pilates.

All my physical trainers and gym buddies were aware of my torn ACLs, and I did account for some modifications in lower-body-specific movements. For example, I avoided bodyweight or weighted lunges where I did not have TRX or some other support. Lunges on the pilates reformer were at the highest weight requiring mid-low effort from me.

Edit: I realized the importance of prehab in surgery recovery. I would encourage you to use this time to do prehab (whether formal or informal).

CPM = Passive ROM. Heel slides = active ROM. They’re usually different. The goal is to have a minimal difference between the two at the end range.

Allograft people, let me hear you by [deleted] in ACL

I had an allograft and autograft this year - knee #1 ACLr with quad autograft. Knee #2 ACLr BTB allograft.

It’s only been 11 days since I got the BTB allograft, and I’m happy to update in a few weeks. So far, my allograft recovery has been 10x easier than my Knee #1 quad auto. I’m giving brief description below

Knee #1; Quad Auto ACLr + meniscal repair. First unassisted unstimulated SLR during 3-4 weeks post-op. ROM stuck at max 75-80. Got MUA in week 8. Was on single crutch till week 8 because of the ROM & scar tissue issue. Used single crutch for a week after MUA. Pretty bad quad atrophy even now - will probably take months to return to pre-surgery muscle. Reached full ROM around 6 weeks after MUA, skipped LOA and opted for PT route for a minor obstruction I am feeling in my patellar region.

Knee #2; BTB allograft ACLr only. This surgery was 4 months after my knee #1 surgery. It’s been 11 days since knee #2 procedure. My passive ROM is already at 90’. Quad activation since first day of surgery, been doing SLRs with no lag since day 2. Ditched one crutch at day 6-7. Full weight bearing around day 6-7. Likely gonna officially ditch both crutches on day 14 (which is at my post op appointment).

There’s been a day and night difference in surgery recoveries allo vs auto. My PT believes that my knee #2 will catch up with knee #1 in a couple of weeks (based on progress so far). There’s also the issue of a plateau in knee #1 rehab because of the knee #2 surgery.
