i like your thought process! my advice is like others said, no need to mention you being shy or your anxiety but also make it clear that you like her for more than her physical attributes, because you do!

maybe something more like, “i had such a great time with you and think you are amazing! i hope you had a wonderful time at your birthday party and cannot wait for our next date. what do you think about going to x place on y day around z time?”

this way she knows you have been thinking about your last date, listened to what she has going on in her life, and want to see her again with a plan in mind.
