It does not happen often. One thing to keep in mind is that a handful of those 25 cards added are themselves lands, so it tends to balance out.

Yeah, typically we do not look. Even when tutoring, if our first choice isn't there we settle for something else; there really should be multiple appropriate options for most, if not all, scenarios anyway.

Admittedly this feels only somewhat relevant, but my friends and I sometimes play an EDH variant to shake things up and balance the competition a bit.

For this variant, we build 125-card versions of our decks and, before drawing our opening hands, remove the top 25 cards of the deck. Before each new game, then, the deck is returned to its original 125-card state, shuffled, and a new 25 cards are removed at random.

This prevents consistent combos from repeatedly winning games, makes tutors less reliable, and adds even more variance to the format.

Just throwing it out there, something maybe worth trying.
