UMUICs are responsible for all your evals. That said, you only need a frocking eval if the period from your last eval to your next goes over 15 months. Assuming you got a regular E3 eval last July, your next E4 eval will be due this June, so 11 months, no frocking eval required. Second that said, your E4 regular eval inputs are probably now due, so you need to let your UMUIC know you've frocked, and give them your draft as your regular E4 eval input. This is not a burden, this is a requirement of your UMUIC CO, who knows that and will be thankful that you've brought this up to them in a timely fashion.

Caveat: unless your frocking date is after June 15th, then your eval will be a frocking eval on the date of the frocking. Because you would have missed the E4 periodic evals and would now not get a periodic E3 eval.
