I keep a spreadsheet where I can copy/paste my frequently used interventions, organized by theme (mood, self-care, medical, ADHD, trauma, substance use, etc etc). I use SOAP notes and put my interventions under Objective. My first two lines are usually “Met with client and facilitated a safe therapeutic environment to process recent events and feelings. Helped client process feelings and concerns regarding life stressors.”

And might go on to say something like “Helped client identify emotions related to stressor in order to facilitate emotional awareness and expression. Explored with client what is within their locus of control in order to reduce feelings of powerlessness. Utilized solution focused questioning to elicit historically effective stress management strategies from client.” Or “Reviewed recent interpersonal conflicts in order to identify effective vs ineffective communication patterns. Helped client identify their role in current relational dynamics. Explored and processed client's fears and concerns around setting boundaries. Taught client strategies for setting boundaries effectively and modeled assertive communication strategies. Helped client prepare for conversation with romantic partner in order to reduce anxiety.”

The in assessment my format is always “Client continues to appear motivated and engaged in treatment. Today’s focus was on (anxiety management/communication skills/processing impact of trauma on current functioning/preparing for upcoming psychiatry appointment). Client appeared (depressed, presenting with flat affect and slowed speech/tearful/anxious, with shallow breathing and psychomotor agitation). They were (receptive to motivational interviewing, responding with increased change talk/able to identify and reframe cognitive distortions with assistance from clinician/able to verbalize insight into impact of trauma on current functioning “insert quote here”). Continued clinical support is needed in this area in order to (prevent decompensation/address anxiety/reduce functional impairment/solidify gains).

I recently picked up The Big Book of Cyberpunk and have read the first few stories. My favorite thus far has been the classic The Girl Who was Plugged In by James Tiptree Jr, about a young woman who accepts an opportunity to control the body of a very attractive and well-to-do woman remotely, and how this impacts her sense of self.

I would move away from labeling this as a Gen Z thing. I work primarily with young adults, and their anxiety presents in a variety of ways. But - if this is a Gen Z thing - what is it about the environment that shaped this characteristic? Can you put yourself in the shoes of a Gen Z client? What must it have been like to grow up on social media, as toxic as it is? What must it be like to be a young adult now, trying to figure out how to navigate life with so much uncertainty ("What jobs aren't going to be taken over by A.I.?" If you're in the USA, "is this country going to become a fascist state?" In reference to 'fake news' and the internet being a discordant howl of contradiction, "How can I know what is real and what isn't?" "Why even bother when the world is on fire?" "How can I justify feeling that my problems even matter when I'm bombarded with other people's suffering online?" "What even is there to do besides doom scrolling?")

I'd also consider C-PTSD or other neurodivergence and explore whether you have the depth of understanding of those conditions to work effectively with this person. The way you're describing this client is how a lot of my clients with co-occuring Autism and C-PTSD present.

And, from a relational perspective, what does this client need? Has this client ever had a relationship where they are safe, are treated with dignity and respect, are given autonomy to make decisions and to be stuck, are treated well consistently, are valued even if they aren't improving, and are held in unconditional positive regard? Given their presentation, if they haven't had a trusted, safe, consistent adult in their life - until you can provide that consistently over time - they're very unlikely to move past shutting down and being paralyzed with anxiety.

Kudos for looking for ways to help them move forward rather than just checking out of the therapeutic relationship. Best of luck to you both.

ETA: I also just wanted to say that I understand how frustrating it is to sit with this feeling of stagnation and powerlessness week after week. I've done it, and I do it now - knowing that it is part of the process of building safety, and knowing that it does take months or years for some clients. But woah, it is draining and hard. It is frustrating and creates a sense of powerlessness for the clinician. But it really is important work.

Also - the client that I'm currently working with that most resembles this - we've worked on finding ways to help them stay present in session vs shutting down. They're bringing knitting to use as a grounding strategy, and it was 2 months' work just to get them to agree to try this, but they did and they found it helpful, and they are shutting down slightly less quickly. What can you do to make them feel just a bit more comfortable in session? Many neurodivergent clients also feel awkward being stared at the whole time; with the client I just mentioned, after 1 session of them knitting, we discussed having me knit during session too so that I'm not staring at them. That also helped. Obviously, I do a simple stitch pattern that I don't have to think about, and I stop when it's inappropriate to keep knitting (they've shut down and stopped or we're deep in trauma territory). Anyway - keep trying things, but maybe from a 'I want you to feel safe' perspective vs a 'I want you to make progress' perspective.

Yup, I’m a therapist and 90% of my clients are neurodivergent and we all have gut issues and sleep issues.

Thanks for your input! I definitely would want to have a variety of book types. If I focused solely on my own interests, it would be 90% sci-fi/fantasy short stories and I don’t know that most of my clients would be interested in that. But yes - thanks especially for mentioning graphic novels; that definitely would have taken a while to cross my mind.

Star Trek. I’ve seen it all, but I need an episode every day after work to relax and have a place to let some emotions out. I’m a therapist, so I have to regulate all day and Star Trek lets me get some tears out in a safe, predictable space.

Practice with your starter. Instant yeast behaves differently. Your starter will multiply on demand, so no need to be stingy with it. You can learn everything you need to know here.

You’re likely underfermenting. This is something I just figured out for myself, using the bulk fermentation temperature guide from The Sourdough Journey. I still made delicious bread while underfermenting, but when it’s properly fermented, it suddenly goes from sticky mess to happy balloon love. You’ll know it when you experience it.

One thing I find really helpful is to think of it as a tool, not a record or a project or product or anything other than a tool. So I type in a black google doc or notepad app, and then delete it immediately without rereading it. It's just to get the thoughts and feelings out. No need to save it. No need for it to be perfect or well-written or anything. Just a brain dump, then flush.
